I recognized the potential for injuries that occur in dance and any activities involving movement and dance such as those taught at D & J Dance Company. I understand that catastrophic injury, paralysis, or even death can result from improper conduct activity. I hereby consent to the above named students participating in activities on equipment owned and/or used by D & J Dance Company and hereby agree that I, for myself, my child(ren) adopted or otherwise, my heirs and executors, waive and release any and all rights and claims for damages that I may have at any time against the studio or its agents and representatives for any injury or damages in connection with my association with or entry in dance or other activities sponsored by D & J Dance Company. I understand that during activities sponsored by D & J Dance Company my or my child(ren)'s image and/or voice may be recorded. I grant D & J Dance Company permission to use the sound, still, or moving images in any medium for educational, promotional, advertising, reproductions, and/or other purpose that supports the mission of D & J Dance Company.
*Students should strive for perfect attendance year-round. For students to participate in any performance they must attend regularly.
*Please notify the studio as soon as possible if a student will be absent from lesson.
*The are no credits given for missed lessons.
*Make-up classes are offered as a courtesy; availability cannot be guaranteed. Make-ups expired after 30 days from absence.
*No make-up lessons are offered during the 30 days before any performance unless approved by D & J Dance Company.
*Dancers excessively absent from class within the 60 days prior to the show may be removed from he show at Directors discretion.
*Long term absences (more than 3 weeks) without motive will automatically forfeit a student's place in a class and account holder will be required to pay for all missed months.
*The studio lesson schedule is subject to change.
Tuition is due promptly on the day you become a student. A late fee of $25 is automatically added all accounts owing tuition the day after your due date. The studio does accept: Visa, MasterCard, American Express or Discover. Check and cash are also accepted form of payment. A $25 fee is added to all bounced checks. Statement will be only passed out in class or emailed it to the responsible party if tuition is late. On the 15th day of the past due, services are suspended until tuition is paid full.
"An annual registration fee will be collected at time of enrollement. Tuition payments are non-refundable. Students may transfer from one class to another as each class is age and level appropriate. For illness, injury, or family emergency, a drop may be requested in writing. A $25 drop fee may be assesed and the resulting financial difference will be awarded tas a credit on account.
Payment plans are available: all registration fees & tuition fees are due the day a student register. Tuition payments must be set up with a credit card or debit card for automatic withdrawal. To by pass the automatic withdrawal a payment by cash or check maybe processed with our front desk staff on or before your tuition due date. "
• No food or drink in the dance room, besides water.
• No snacking during class time.
• Cell phones are not to be used in class and should be turned off or on mute.
• Profanity, unsportsmanlike conduct, and or gossip will not be tolerated. Positive energy only!
• Keep hands off the mirrors.
• No hanging on the barres.
• Students and parents will conduct themselves in an appropriate manner while at D & J Dance Company and at all performances and events in which D & J Dance Company is being represented. Proper respect for instructors is expected of all students, parents/guardians and siblings.
• Bring a good attitude to class. Attitudes that are disrespectful, uncooperative, or aggressive do not have a place at D & J Dance Company.
• Students will not speak negatively about one another or the teachers.
• Any parent/guardian or student who disrupts class or approaches a teacher, another student or parent with the intent to cause dissent or contravene the policies and procedures of D & J Dance Company will be dismissed immediately.
• Any inappropriate or slanderous remarks about D & J Dance Company on any social media site will be cause for immediate dismissal from the studio.
• Always use strong arms, stretched feet, proper body alignment and 100% of your energies during class.
• And remember to smile, learn, and have fun in each class!
Query & Complaint Procedures
We appreciate that parents need to talk to teachers about their children but this may not be during class time. Please make a time outside class time when the teachers are free.
If your query about your child is lengthy or of a private nature please arrange to make an appointment with Joe Orta or Daphney Yalung. Requests can be made in writing on our complaint form or by emailing email@example.com or please advise if you need a specific teacher at the meeting.
If you wish to lodge a complaint about another member of the studio or about our studio, please email or write to firstname.lastname@example.org or fill out the complaint/suggestion form.
All complaints and queries will be dealt with promptly, seriously and with sensitivity and remain confidential. We appreciate all your suggestions, as we strive to have a successful healthy dance studio.
*All D & J students are given the opportunity to perform.
*Parents are responsible for purchasing necessary costumes from D & J Dance Co. by the due date.
*Costume payments are non -refundable (minimum of $50 will be collected depending on how many pieces you or your child is performing)
*Concert fee of $110. Additional class for costume fee is $50. All fees are non-refundable. There will be a late fee of $50 for any costume fees that will be made after the given date of costume fee due date.
*Concert fee breakdown:
$50 includes two tickets! This will accumulate expenses for space rental, lighting company, and any other materials needed for the concert.
$60 Costume fee, tights and shoes are not included.
*At Director's discretion a new student may be invited to perform regardless of enrollment date.
*Any dancer who is absent within 60 days prior to show can be removed from the show at directors discretion.
*If a student quits two weeks or a day before performance or recital, please remember the whole dance & choreography will have to change. This is not fair to the other students in the class.
As we get closer to our June/ December performance, we will send out the rehearsal schedule. Rehearsals are held the week of the show. During this week there will be no regularly classes scheduled classes unless the class is not part of the show. Students are expected to attend all rehearsal and stay for the duration for the scheduled time as this is the last chance to clean up, make changes, and polish the dances. Even when one person misses, they an upset the balance of the spacing. If you are not present at rehearsal, there is a possibility that you will not be in certain parts of the dance and in some cases removed from the number.
* Class start time may be changed if there are 4 or less students enrolled.
* The Studio Director/Instructor has discretion to require that a student observe a lesson for appropriate reasons.
*Please be prompt in dropping off and picking your student.
*If a student is 15 minutes late he/ she will not be allowed to take the class because it will disrupt the whole class.
* For their safety, all students must be escorted into the studio before lessons and wait inside the studio to pick up after lessons.
*No chewing gum in the studio at anytime.
*No food or drink allowed in the studio(water ok)
* Parents are responsible for monitoring all sibling not in lessons
*Parents are not allowed in class unless permitted with instructor. This will allow students to be attentive and not be distracted while the instructor is teaching.
*D & J Dance Company and staff are not liable for any damage to or losses of personal property or valuables.
* Any choreography learned by a student is the sole property of D & J Dance Company and may not be copied or used for any activity outside of the studio. Permission must be granted by the studio for any student to use studio choreography at any venue where the member is not with the studio or acting on behalf of the studio.
*All imagery including, but not limited to, photographs and video recording of a student is the sole property of the studio and maybe used at the discretion of the studio for advertising/promotional purposes.
*it is the responsibility of the parent or adult student to inform D & J Dance Company any changes of enrollment. Fill out the add/drop/withdrawal form 7 days prior to your billing date.
*D & J Dance Company has the right to refuse service to anyone.
*Registration form & waiver of liability must be completed for every students before enrolling in any lessons or participating in any trial or drop in classes.

Each student is encouraged to make up missed classes in any classes he/she want to take within 30 days.
*Withdrawing or holding Policy: You must complete a drop form of thirty-days prior to your upcoming tuition payment. You will be responsible for the next payment if you don't give a thirty-day written notice. Please be aware the no prorating or refunds will be issued due to withdrawing from classes.
*The students who are under 18 yr. old will be asked to register with the parents.
*Any concerns /suggestions about the studio must fill out our concerns/suggestion form; you will be able to submit to our front desk person.
We appreciate your concerns and suggestions, this give us time to review and fix any problems. Any misconduct or inappropriate approach will not be accepted.
*D & J Dance shirt should be bought at registration.
