I understand this registration contract represents a financial commitment for the classes I have indicated. Refunds are only given before the first week of classes. Four make-up classes are allowed per session and must be completed during the same session. I certify that this student is in good health and capable of participating in all classes. I agree that I cannot hold SMAPA or any agent, representative, faculty member, or employee liable for injuries or illness contracted by the student while participating in SMAPA‘s activities, online or in the studio. I also agree that I cannot seek indemnification, reimbursement, or any other type of compensation from SMAPA, a faculty member or employee for any injuries or illness contracted while a student/participant of SMAPA. I understand that SMAPA cannot be responsible for any lost or stolen items.

In the event of a medical emergency, SMAPA has my permission to authorize first aid and /or lifesaving treatment to the participant listed above. In the event of an emergency, every effort will be made to contact me immediately. If I cannot be reached and emergency care is necessary, SMAPA will not be responsible for any medical charges incurred.

The above named person (or guardian of same) does hereby give approval to participate in any and all activities and assumes all risks and hazards incidental to the conduct of the activities including transportation to and from the activities if necessary; and do further release, absolve, indemnify & hold harmless SMAPA, the organizers, the sponsors or any of the supervisors appointed by them. In order to meet all legal requirements, the above named person (or guardian of same) gives consent for any and all necessary emergency medical care as such arises. It is advisable to consult your doctor before participating in any type of aerobic program.

I understand that dance class participants may be photographed or videotaped during dance classes and summer programs. I understand that these photos and videos may be used for marketing purposes in print and online promotions for SMAPA.
