I understand that in addition to accepting this policy, I will also email/mail/bring in a hard copy with the student's parent/guardian signatures.
Waivers and Policies ● 2018/19 Season
1) I attest that my student and my parent/guardian information is correct in Master Ballet Academy's DanceStudio-Pro. This includes email addresses, phone numbers, at least three (3) emergency contacts, mailing address, medical information, etc.
2) By agreeing to the terms, I accept responsibility for each point below and will also sign a waiver in person.
All Parents (or dancers, if not a minor) must sign before attending any Master Ballet Academy Classes. You'll need to sign in-person and also agree online here.
I, the undersigned, agree that the above information is correct. I recognize and understand the risks of physical injury inherent in dance training and I am willing to assume those risks. I will not hold Master Ballet Academy aka Nina Marlow School of Ballet, LLC. or any faculty member, employee, guest instructor, or volunteer liable for injuries sustained or illnesses contracted by the participant while in attendance and/or participating in classes in the above mentioned school. I understand that the student may be physically touched during dance training and/or dance instruction. I will not hold Master Ballet Academy and its faculty members liable for physical touching which is incidental to and in the course of dance instruction and training. I agree to indemnify Master Ballet Academy and its faculty members and volunteers of both for all liabilities, costs, and judgments arising from acts of omission committed by me or my child which may result in injury or damage to any person or party.
Photo/Media Release
The studio may use my/my child’s name, likeness, video or photo in marketing, promotional and advertising pieces, including (but not limited to) Master Ballet Academy and Phoenix Ballet's printed materials/ collateral, website, facebook, instagram, twitter, youtube, etc.
Emergency Release
In the Event of an emergency if parent/legal guardian or the emergency contact listed is not available I give consent to Master Ballet Academy aka Nina Marlow Ballet School LLC. to render treatment if necessary and take action as necessary for the health and well-being of my child/children. I affirm that I hold a valid personal health insurance policy sufficient to cover any and all circumstances that may arise from participating at Master Ballet Academy.
Level Placement
Students will be placed in levels according to their age and ability. Placement is at the discretion of the Faculty and subject to change by the Directors at anytime.
Attendance
Regular attendance is important for the learning process. Students are expected to attend every class for which they are enrolled, and should arrive 5-10 minutes early to prepare for class; this is important for proper warm-up and stretching. Students consistently late or skipping class may be prone to injuries. Consistent attendance will be a factor in level placement throughout the season. Depending on the class and the instructor's discretion, students may not be admitted to class if more than ten minutes late. If late, students should not enter the class in the middle of a combination. Please wait until the music is off to enter the studio. Regular attendance to all classes a student is signed up for is important to their continued success with MBA.
Make Up Classes and Refunds
Make up classes are only allowed if the absence was due to illness or injury. A doctor's note is required.
All MBA tuition is nonrefundable.
Tuition
Tuition is non-refundable and students must pay for classes before taking.
We accept Checks, Cash and all major Credit Cards (all card transactions will be charged a convenience fee of 2%)
All payments are due on or before their due dates. If payments are not received by the due date, after a 7 day grace period, a $35 late fee will be charged. If payment is not received after 7 business days, the student will not be allowed to take class until tuition is paid. Tuition is based on the number of hours students sign up to take each week. It is the responsibility of the Parent/Guardian to make sure payments are made.
The Nutcracker and Spring Performance participation, Master Classes, Summer Intensive, Private Lessons and anything besides classes in each level are not included in tuition. Please note that Partnering, MBAPC and the Pre-Professional program have separate tuition/fees.
Payment Options
By registering for MBA classes, you agree to commit to an entire season of classes, August - May, and the payment of the full season. While MBA is happy to offer payment plans for your convenience, including 2-installment plans, quarterly and monthly options, some quarters/months may have less class days than others. However, tuition is based on the overall entire season. Therefore, MBA does not offer pro-rated payment options for quarters or months based on number of classes. You agree by registering to pay in full for the entire season. The only exception being an illness or injury with a Doctor's note and MBA Directors approval.
1: Quarterly - tuition to be paid in 4 installments
2: Two-Installment (save 2%!) - tuition to be paid in 2 installments
3: Full Season (save 4%!) - paid in full at time of registration or by August 13, 2018
4: Monthly - 10 installments, includes a convenience fee of $20 extra per month added into price already
Sibling Discount
The highest tuition is paid in full. Each additional child receives a 10% discount off their tuition. Boys are not included in the Sibling Discount, as their tuition is already discounted (30% off.)
Spring Performance
MBA's goal is to offer the best in dance education to their students. Performing is an integral part of this process. Therefore, by registering your student, you agree to have them participate in the Spring Performance show. The fees for the performance cover the cost of the instruction for the piece, costume rentals, costume fittings and other instruction as deemed necessary by the instructors. For levels I-III the fee is $210 , for levels IV-VII the fee is $280. Payment is due by January 7th, 2019
Partnering Class: For the safety of all students and teachers, female dancers may not weigh more than 110 pounds to be lifted. They will be allowed and taught to do other forms of partnering including pirouette, promenade, dancing and choreography, just not lifts. For male dancers, if they have not developed the strength necessary to properly lift, they will not be allowed based on director decision. This is strictly safety purposes.
Private Lessons
Private Lessons are not required and are not included in tuition. They are an additional fee.
Please note: if you have to cancel a Private Lesson, a 48 hour notice is required or you will owe a $25 cancellation fee. If you give less than a 24 hour notice, you will owe the full lesson amount unless you have a written doctors note or an emergency as approved by the directors. When taking a private lesson (including choreography) with a non-MBA faculty member, studio room rent must be paid, $25 per hour all studios except Studio C and E are $15/hour*.

Competitions: Competitions are not required but if your student is doing a competition regardless of location, and wish to have a faculty member on site with them as a coach, all accommodations (travel expenses, per diem and hotel) must be paid by the student's parents/guardians for the Instructor(s) who will be attending competitions with your child. In order to register for a competition under the Master Ballet Academy name, students must have the director's approval, if not, they still pay to compete, just as an individual.
Parent/Guardian Guidelines: Parents, guardians and family members are welcome inside the main lobby. They are not permitted to watch class or private lessons without the approval of the teachers. Doorways need to be kept free of people due to fire safety regulations. In the chance that adults are allowed in studios, shoes are not permitted. Parents are asked to respect the studio rules below and also act as role models for the students. Family members, friends, etc. are not allowed to run around the lobby, make loud noises or other distractive behavior.
Studio Rules
Students are expected to be on time and respectful to their instructors, employees, guests and each other. They are expected to focus on their hard work during class and have a positive attitude. Disrupting classes is not only rude to the instructor, it interferes with proper learning for everyone in class and may cause injuries. If any student is disrupting class in any way, there will be a verbal warning. If the disruption continues, the student will be asked to leave class, and a meeting with the parent/guardian will be scheduled. If disruptive behavior persists, the student may be asked to leave the studio.
Please be mindful of the following policies which will be diligently applied in order to provide optimal pre-professional dance instruction. Following these policies will reduce the noise and distractions within Master Ballet Academy, and will help the students focus and achieve their goals.
No parents or non-dancing children will be permitted to watch class except when invited by an instructor, or the dancer has a private dance lesson and/or partnering class. In these instances the participating family is welcome to observe that lesson with the teacher's prior knowledge.
Please extended respect to your fellow MBA dancers and families by:
1) Observing an appropriate noise level in the lobby area
2) Picking up after ones-self, including drinks, snacks, shoes, etc.
3) Supervising all children in the lobby area at all times. Unsupervised children are prohibited without Director's approval.
Please pick up dancers on time upon completion of classes. Should a parent be running late or encounter an issue with pick-up times, please notify the studio promptly to make arrangements. Please be sure your children obey attire and hair policies. Classes, fees and policies subject to change
