Dance Technique Agreement for Waiver
*This is a legal document please read it carefully*
TO: DANCE TECHNIQUE
1. I accept that I will be engaging in physical activity and there is a real risk of injury from various causes including over-exertion.
2. I accept that I should not participate unless I am fit and healthy and, where appropriate, I will seek medical advice.
3. I accept all risks resulting from my participation which may result in personal injury and I release Dance Technique and its teachers/employees from all claims and proceedings arising out of my participation and indemnify them against all claims that may be made arising out of my participation.
4. In the event that I am injured I consent to Dance Technique arranging medical treatment and agree to pay the cost of that treatment.
5. I acknowledge that any insurance taken out by Dance Technique may not cover me for all the injuries I may sustain.
6. I will inform in writing all my known medical and physical conditions that may be relevant if medical treatment is needed and I warrant that I am fit to participate despite those conditions.

Dance Technique Photographic/Filming Consent Form
Dance Technique wishes to record images of Dance Technique competitions and programs for promotional purposes. These images may be used in mediums including: publications and promotional materials, and broadcast, print and electronic media.
Dance Technique asks individual and/or parents and guardians to provided written permission for Dance Technique’s use of these images on the form below.
Dance Technique agrees not to use any photo in a manner that may be deemed adverse or defamatory to the person signing this form, and it will not use the photo for any commercial gain. The photo will remain the property of Dance Technique and any personal details regarding this photo will be kept confidentially and will not be used for any purpose other than related to this photo.

I give Dance Technique permission to use images taken of me/my children in publications and promotional material, and broadcast, print and electronic media. I agree that I will make no claim against Dance Technique for any payment or fee for appearing in promotional material or advertisement and release Dance Technique from any other claims arising out of Dance Technique’s use of the images taken of my child or myself.

Dance Tech BAND APP Groups Consent Form


Dance Tech has a private studio app called BAND, this app has a number of closed groups for each age group of students. In these groups, we post messages and choreography videos, that we would like students to practice in between classes each week.

This APP is monitored daily and only those accounts approved by our admin team are allowed access to the BAND APP.

The app has been designed to allow parents to monitor their child's activity by setting parental supervision.
This is turned on in the Settings tab. Refer to the help guide on the BAND app or the official BAND website.

I give consent for myself or for my child to join & participate in the private, closed BAND app groups.
