I understand that risk of injury is inherent in any physical activity and I, on behalf of myself and my child, knowingly and voluntarily accept that risk. I, the undersigned, for myself, my heirs, administrators, and executors, hereby waive and release any individual staff member and Stone Dance Academy, LLC from all claims or damages of any kind arising out of my child’s participation in the exercise and/or dance program of Stone Dance Academy, LLC. I understand that dance classes may include, without limitation, dancing with props, stretching, barre work, across the floor combinations, dance routines in the center, and other related activities. I further understand that all the activities of the dance class involve some degree of risk of strain or bodily injury. Stone Dance Academy, LLC is not responsible for personal property. I further certify that the student is in proper physical condition to participate in the exercise/dance program and that he/she has been examined by a licensed physician and found to be in proper physical condition to participate in said program. I, the undersigned, do hereby authorize Stone Dance Academy, LLC and its designated agents (being teachers or administrators employed by Stone Dance Academy, LLC) to obtain medical treatment for my said child in emergency situations where I cannot be reached in time to authorize the treating physician to provide such emergency medical services. I understand that I am responsible for any medical expenses and that the absence of health insurance does not make Stone Dance Academy, LLC responsible for payment of medical expenses. This authority includes the power to authorize all treatment deemed necessary under the circumstances by a licensed physician. This power is a power of attorney and shall remain in effect for one year from the date accepted and registered.

I agree to be responsible for reading studio correspondence and respecting deadlines. I hereby acknowledge that I have read the statements in our Terms and Conditions and agree to participate accordingly. I acknowledge that all paperwork must be signed and first month’s tuition and registration must be paid before participation in class.

As with any program, steady attendance is a requirement for success. Failure to meet such requirements can lead to adjustment in level.
Guardians are responsible for communicating absences to the dance studio director or dance instructor directly. Any student who misses 3 consecutive weeks or more may be required to forfeit their class space. Excessive absences could result in removal from the recital piece. After March 1st, if a student misses more than 4 rehearsals, they will not be permitted to be in the recital. Dress rehearsals are mandatory for recital participation.
Make-up Classes: Make-up classes are permitted so long as you have management’s approval and you are making your class up in the same level your dancer is enrolled in. For example, Primary students who miss class can attend another Primary level (doesn’t matter the genre of class), in order to make-up for their missed one.

The following are considered absences:
-Death in the family
-Scheduled school programs
-Wedding or religious dedication of you or a member of your family
-Serious illness or infection with a doctors note
-If a dancer is injured or mildly sick, they will watch their company rehearsal and take notes.

If a dancer is injured or sick, you are required to still pay tuition to hold your dancer's spot in the class. Make-ups may be offered at the discretion of the owner.

I understand that one make-up class is permitted for each class my child misses, apart from holidays or days that the studio is scheduled to be closed. Make-up classes must be taken within 30 days of missed class(es) and require permission from teacher of that class for the scheduled make-up. I also understand that all fees paid are nonrefundable and nontransferable. Payments for months with holidays or closures will not be prorated or changed as the payments are already defined by the number of weeks in the dance season. The parent or guardian is responsible for notifying, in writing, Stone Dance Academy of any change to the credit card or checking account. All accounts must be paid in full in order to participate in recital. Should this provision have to be enforced by legal means, the undersigned person(s) is responsible for payment, as liquidated damages, the costs of collection, plus interest at the legal rate and reasonable attorney's fees as determined by the Court or 15% of the amount collected failing such determination.

Stone Dance Academy is committed to providing a friendly and supportive atmosphere for all students and we ask that families and dancers help us create a positive dance experience. We ask that dancers and parents maintain a respectful attitude always.
Dance Conduct Guidelines:
1. Please show up to class prepared following our dress code and ready to participate.
2. Please log in to your portal and log any absence for the week before class.
3. No cell phones, gum chewing, candy, or eating in class. Water bottles are recommended for all ages!
4. Please pick up after all members of your family in every area of the studio that you are in during your visit.
5. Observers, parents and friends shall remain outside the classroom during classes. If you are observing, please do so quietly or you will kindly be asked to wait in the lobby.
6. Please use email during non-teaching times to communicate with us, as teachers do not always have time to speak with you between classes.
7. It is your responsibility as parents and students to be fully aware of all dates and times for rehearsals, dress code requirements, and necessary shoes for your dancer needs. Picture Day, Dress Rehearsal and Recital means no jewelry (including earrings), no nail or toe polish, no temporary tattoos, etc. The information will be emailed to you, is on our website and many times on our bulletin boards throughout the studio. Missing the technical, blocking and dress rehearsal makes it impossible for a student to participate in recital, therefore, attendance at such rehearsals are mandatory.
8. We have ZERO tolerance for bullying, name calling, threatening, mistreatment of any kind, or physical interaction that is not within the realms of the dance class. If a student is suspect to any of the listed items above, parents will be notified with a warning for the first infraction. The second infraction will result in a temporary suspension, and the third infraction will be cause for removal from the studio permanently. In such cases, we hold the right to skip any of the above steps and ask for students to be removed permanently based on the nature of the infraction.

As part of the curriculum at Stone Dance Academy, the studio has implemented a dress code policy that all dancer’s must follow. Dance is a discipline and begins with the way a student dresses for class. By dressing the part, a student looks like, behaves like and becomes a dancer. A neat and tidy appearance sets an attitude of attentiveness and respect for students, teachers and the art of dance. Teachers must be able to see a dancer’s alignment, lines and positioning from head to toe in order to make the proper corrections when needed.

If a dancer is continuing to come to class in the incorrect uniform, the following procedures will take place to follow our proper protocol:

1. The first offense is a verbal warning.
2. The second offense is a written warning.
3. The third offense is sitting out of class.

If after the 3rd time the uniform policy is still being neglected, the dancer will continue to sit out of class and potentially be unable to perform at the year end recital.


SDA has worked to create a dress code specific to each class that ensures the teacher can adequately see the skills demonstrated. Please use the following outline to determine your dancer’s required uniform for class.

Primary
Primary students are required to wear the Bloch pink leotard, pink tights, and pink ballet slippers. For Tap, they can add black shorts to their outfit and will be required to wear black Mary Jane tap shoes with bows. Tumble is the same as tap but with no shoes.

*Hair bows of any color or style are permitted.

Pre-Academy
Pre-Academy students are required to wear the Bloch lavender leotard, pink tights, and pink ballet slippers. For Tap, they can add black shorts to their outfit and will be required to wear black Mary Jane tap shoes with bows. Jazz and Lyrical are the same as tap but with tan colored jazz shoes and tan colored lyrical shoes. Tumble is also the same as Tap, Jazz, and Lyrical, but with no shoes.

*Hair bows of any color or style are permitted.


The Academy
All Academy students are required to wear the Theatrics leotard color for their corresponding level. The correct color leotard, tights, ballet skirt, and black shorts are a requirement for these classes. Other kinds of skirts and different black shorts are not permitted.


Acro
Acro requires our studio approved Theatrics black leotard and our studio approved Theatrics black shorts. Tights are optional in this class but must be pink if they choose to wear them. The studio also allows black leggings so long as they are solid black with no designs. If a student is enrolled in The Academy, they may also wear their Academy leotard instead of the black one. *We prefer everyone in all black*

Tap
Tap requires our studio approved Theatrics black leotard, our studio approved Theatrics black shorts, pink tights, and black lace up tap shoes. If a student is enrolled in The Academy, they may also wear their Academy leotard instead of the black one. *We prefer everyone in all black*

Hip Hop & Musical Theatre
Both Hip-Hop and Musical Theatre are allowed to wear any clothing style of their choice so long as there are no midriff baring tops, inappropriate length shorts, or any other revealing clothing. Shoes must be white sole sneakers for Hip-Hop and Musical Theatre must have tan jazz shoes.


Please note our policies for Picture Week, rehearsal and Recital:
No nail polish, earrings, rings, necklaces, watches, fake tattoos, hair clips that are not part of the head piece, scrunchies or bracelets.

Stone Dance Academy requires a 30 day written notice (1 month) if you choose to drop out of classes. (For example, if you drop out during any point of the month, but have not given notice, you will be required to pay for that month's tuition.) We are unable to give refunds. (We will make exceptions if there has been an injury of some kind, supported by a physician's note.)


All camps, intensives, and workshops are non-refundable.

Students that become injured at SDA or elsewhere must notify our staff and provide written doctors note as to the extent of the injury. In the event that an injury will require prolonged healing time (4 or more weeks), the student may need to be removed from choreography, at the discretion of the staff.

In the event that a student sits out of class due to injury, they will be required to observe for the remainder of the dance day, unless immediate medical attention is necessary, and they are picked up by an adult. Frequent occurrence of sitting out may indicate overall health and wellness issues of the dancer and may require removal from the class. We recommend seeking medical attention after sitting out for any injury.

Please refrain from bringing your child to dance if they are sick with a contagious illness. SDA provides hand sanitizer, daily cleaning, and encourages our staff and students to take care of their health.

Dance classes are often competitive, and involve constructive critique from our qualified instructors. In the event that a dancer is struggling with significant emotional or mental health related challenges, please seek professional help and communicate this with SDA staff. In the event that these challenges are affecting the student’s ability to accept and apply instruction/critique, or their behavior is affecting the culture of the classroom, we may choose to remove the student from the class.

Each class that is labeled for recital participation will have additional fees for costuming and recital showcase. Company costume fees are due earlier in the year as we attempt to get them exposure and experience throughout the year by way of performances, conventions, and possibly competitions. Shoes, tights, accessories, and makeup are all part of the costuming and will have to be purchased before any performance or recital. Recital fees are determined once the location is secured each year and are due after January 1st of that year. By signing this you are acknowledging your responsibility as the parent/caregiver to pay all fees associated with the recital. Recital fees and costume fees are non-refundable. Participation and attendance is vital in each class, especially before recital, as we have a lot more to cover and choreography to complete. By signing this, you are also acknowledging your responsibility as the parent/caregiver in making sure your child is "show ready" every time. This means no jewelry (including earrings), no nail or toe polish, no temporary tattoos, etc.
*We understand many girls get their ears pierced for birthdays, holidays and/or summer, but please make every effort to get ears pierced after recital or in plenty of time to be able to remove for recital/picture day. We want all girls to look uniform on stage and it also causes safety issues with costume changes, hair styles, etc.
