I hereby give permission for my child/ myself (if over 18yrs old) to participate in classes conducted at Vibe Dance Studio.

I fully understand that participating in these dance classes is at my child’s/my own risk. Whilst all necessary precautions will be taken to ensure the safety of every student, no responsibility will be taken by the studio or anyone involved with the studio.

In the event of a medical emergency (when the emergency contact cannot be contacted), I give permission for staff of the dance studio to seek medical assistance for my child/ myself and/or arrange for emergency transportation for my child/ myself to the closest medical facility.

I acknowledge and consent to Vibe Dance Studio using photos/ videos of my child/ren for the purpose of advertisement, marketing, signage, social media platforms (such as Facebook and Instagram), the BAND app and the studio's website.

I understand that names of children are not used in any of the aforementioned instances unless additional consent is expressly given by the child/ren's parent.

I acknowledge and understand that there is no financial compensation for use of my child/ren's photos/ videos.

I acknowledge that I have read and understood Vibe Dance Studio's terms and conditions as displayed on the Vibe Dance Studio website.
