Health Information and Medical Release/Waiver Form I, _________________, the parent/guardian of _________________________,acknowledge that participation in dance is potentially dangerous and there is an inherent risk of injury involved. In allowing my child to participate in Border Danceworks activities, I hereby assume all the risks associated with the performing arts. I understand the importance of myself and my child following the instructions and rules set by their instructor/s, and I agree to release Border Danceworks and it’s employees of any and all liability which may arise as a result of my child’s participation in activities at Border Danceworks.
Students enrolling into Border Danceworks are entitled to one free trial to any style. After their free trial, if the student wishes to continue they must complete an enrolment form and pay the full enrolment amount within the first 2 weeks of the term. Enrolments are valid until the end of each school year and must be cancelled in written form by the end of each term when a student is unable to attend the following term. The enrolment fee is non-refundable and any student wishing to leave the studio must give two weeks written or verbal notice to Kylie or Kerry or they will be required to pay until the end of that term. · Payment must be made in full by the end of week 2 unless a payment plan has been arranged. Students are required to pay for all classes they are enrolled in even if they are unable to attend class. If a student is going to be away for extended periods of time (ie. 4 weeks or more) discounted terms fees may be applied for. These arrangements must be made at the commencement of term and discounts will be at the discretion of the Principal. Family discount of 10% for 3 or more classes. Students are required to wear clothing that is appropriate to dance in eg leggings and a t.shirt, Border Danceworks t.shirts are available to purchase. Ballet students must wear a leotard, skirt and stockings (optional) and ballet shoes and hair needs to be pulled back. Border Danceworks uniforms are available to purchase at the studio if you would like one· All our staff are trained and have undergone their Working with Children Check. When enrolling your child in Border Danceworks there is a level of physical touching required when correcting a student's technique or positioning. Border Danceworks staff will treat all students and parents with respect and this is expected in return.
I, the undersigned, give permission for Border Danceworks to use video footage and/or photographs of my child/ren, _________________________________. This usage may include (but is not exclusive to) displaying publicly, distributing, or publishing, photographs, and/or video of my child for use in materials that include, but may not be limited to: - printed materials (eg - brochures and newsletters) - online and offline advertising and promotion - videos and digital images such for use on Social Media. By signing this form, I acknowledge that I am giving unrestricted permission for my child’s image to be used in print, video, and digital media. I agree that these images may be used by Border Danceworks for a variety of purposes and that these images may be used without further notification. I do understand that any identifying information including surname and location will not be used in conjunction with any video or digital images.
