I grant permission for my son/daughter to attend Designed to Dance. My child has no previous or current medical conditions that would interfere or prevent his/her participation.

I authorize that in my absence, if necessary, the participant is allowed to be admitted to a hospital or medical facility for diagnosis and treatment through Designed to Dance. I authorize physicians, to perform diagnostic procedures and/or treatment, for the participant as needed. I hereby authorize D2D's Director to act for the participant according to their best judgment in providing and/or arranging for emergency care in any emergency circumstance requiring medical attention.

D2D holds responsibility to communicating with all dance parents and participants. D2D will provide post notifications via email, text messaging, and selective calls as needed. Parents must have a valid email address and contact phone number on file. All D2D parents are responsible for viewing and reviewing all information once received.

D2D's best point of contact is via email. If calling the studio, the director and/or admin team will get back to you as soon as possible. Digital communication must be respectful and should not offend, intimidate, humiliate or bully another person. Communication must not be misleading, false or injure the reputation of another person.

Designed to Dance (D2D) has put in place preventative measures to reduce the spread of COVID-19; however, D2D cannot guarantee that you or your participant will not become infected with COVID-19. I release Designed to Dance (D2D), the director, its instructors, staff, and all associates from any claims based on the actions, omissions, or negligence of D2D, whether a COVID-19 infection occurs before, during, or after participation in any D2D class or event. I understand the risk of becoming exposed to or infected by COVID-19 and other contagious diseases and viruses even during our safe sanitation/social distancing practices and procedures. I understand that if my child tests positive and/ or shows symptoms of infection, I have the right to keep my child home to quarantine from the studio. I attest to honesty and confidentiality during this time as my child participates in D2D activities and agree to share if my participants immune system has been compromised and/or displays symptoms of a fever or chills, cough, shortness of breath or difficulty breathing, fatigue, body aches, headache, sore throat, congestion or runny nose, nausea, vomiting or Diarrhea and more as the CDC provides information. I understand that there are NO refunds for registration fees, costume, recital fees or classes. I understand that if my dancers class gets cancelled due to weather, sickness, or any issues arising from COVID-19 there is no refund for these missed classes.

This Conduct represents the value we reflect at D2D. Failure to abide by the following information can result in the right to refuse service to any participant for any reason.

Dancer/ Participant agrees to the following:

- Giving 110% to their role as a dancer with D2D
- Attending ALL classes, practices, and performances
- Be on time to ALL classes, practices, and performances
- Wear the proper dance attire (uniform, hair, no jewelry, etc.) while in class, practice, and performances
-Will not wear separate costumes and outfits from the other dancers on their team
- Follow all rules of the dance studio at all times
- Absolutely non gum allowed in class, practice, and performances
- Respect the director, instructors, staff, and peers at all times
-Will not participate in any extracurricular activities that will interfere with my D2D practice time, performances, etc
- Will not exclude my fellow dance mates to form any "clicks"or separate groups within D2D's organization. Dancers understand that we are sisters and brothers who will support each other

I agree to the following:

-I understand that my personal conduct at the studio, performances, etc is to reflect encouragement, promote teamwork & sportsmanship, support for the instructors and director, refrain from any negative remarks within regard to the staff and other participants, and overall ethical behavior.
- I understand that my child must attend ALL classes, practices, and performances
- I understand that my child must be on time to ALL classes, practices, and performances
- I understand that my child must wear the proper dance attire (uniform, hair, no jewelry, etc.) while in class, practice, and performances
-I understand that my child cannot miss class or practice due to homework, projects, conduct, traffic, or hair appointments. (*Mandatory school activity/ obligation is the ONLY excused absence)
- I understand that tuition and additional fees must be paid by the deadline to avoid a penalty
- I understand that all fees paid are non-refundable. No credit will be allowed.
- I understand that if tuition is not paid on time, my child may have to sit out of a class, practice, and/or performance
- I understand that if my child misses class, I still have to pay a monthly fee.
- I understand that parents are not allowed to enter the studio while classes are in session. All classes/ sessions are closed to parents. Parents are only allured to observe when permitted.
- I understand that my child must wear hairstyles assigned to the team. No alterations or complaints because of length or texture of hair and understand most styles will require additional extensions.

I/We (participant included) understand that there are risks associated with the activities provided through Designed to Dance. My participant understands and agrees to voluntarily participate in all D2D activities including any dance training, rehearsal or performance, traveling engagements, and field trips, knowing there is a possibility of physical injury and/or risk. I agree to release and hold Designed to Dance harmless including its Director(s), instructors, staff members, and facilities used by D2D from any cause of action, claims, and/or any legal or financial demands now and in the future. I will not hold Designed to Dance (D2D) liable for injuries, illnesses including known/unknown viruses and disease, and/or any personal injury or any personal property damage ,which may occur on the premises before, during, or after classes.

Dancers of Designed to Dance (D2D) will have opportunities to be photographed or video taped during training and various dance events. I hereby authorize Designed to Dance (D2D) photographer/videographer or project sponsor to use, reproduce and/or publish photographs and/or video that may pertain to the participant. I understand that this material may be used in various publications, recruitment materials, or for other related endeavors. This authorization is continuous and may only be withdrawn by specific rescission of this authorization. D2D or project sponsor may publish materials, use participants name, photograph, and or refer to my participant in any manner that D2D or project sponsors deem appropriate in order to promote/publicize service opportunities. I acknowledge that D2D may choose not to use my photo or video at this time, but may do so at its own discretion at a later date. D2D reserves the right to discontinue use of photos or video without notice.

D2D has converted all major payments to this system. If D2D has any additional fees and/or payments that will be allowed via Cashapp, Zelle, and other electronic platforms, parents will be notified. Tuition payments are due on the 1st of every month. Balances will be updated and posted on all Parent Portals by the 25th of every month. The system does allow payments to be made before D2D post the monthly balance. Accounts where tuition payments have not successfully been processed by the 1st will be charged a $25 Late Fee. Students with a past due tuition balance as of the 2nd day of the month will NOT be permitted to attend class until the account has been brought current, including all past due tuition charges and associated fees.

Designed to Dance does not offer refunds on tuition, registration, or any purchased classes, tickets, and or events.
If for any reason our payment system has overcharged your account, we are responsible for issuing a refund at that time.

D2D will have occasions when we feel it necessary or in the participants best interest to provide and assist with transportation of dancers to and from related events. D2D will take trips to other dance venues, recreational events, attend professional performances and recitals, travel for engagements/performances, etc. In place of individual consent forms, D2D is providing this blanket consent authorization to cover all events. Information regarding each trip/travel event, including location, transportation, and departure/return times will be provided to parents and participants prior to each trip. If there are specific events that you want to opt your participant out of attending or using our arranged transportation, you may notify the director or administrative staff at any time.
