FAMILY COMMITMENT
Prior to attending, I will assess myself and my child for COVID-19 symptoms and other contagious illness-related
symptoms, and if present the individual commits to stay home from all activities.
ASSUMPTION OF THE RISK AND WAIVER OF LIABILITY
The Participant names below (and the Parents / Legal Guardian if Participant is under 18) voluntarily agree to assume all
risks and accept sole responsibility for any injury to (including, but not limited to, personal injury, disability, and death),
illness, damage, loss, claim, liability, or expense of any kind that I may experience or incur in connection with me or my
child’s participation. I hereby release, discharge, and hold harmless North Heights Lutheran Church (North Heights), its
employees, agents, and representatives.
I represent that I have adequate insurance to cover any injury or illness suffered or cause while participating in activities
and further agree to bear the costs of such injury or illness to myself or child. I further represent that I and my Child
have no medical or physical condition which could interfere with our safety in this activity, or else I am willing to assume
– and bear the costs of – all risks that may be created, directly or indirectly, by any such condition.
MEDICAL CARE PERMIT
I hereby authorize emergency medical care or first-aid treatment as needed in the event of illness or injury during a
sponsored activity at North Heights. It is assumed your child is capable of advocating for his/her specific medical and
allergy issues unless otherwise noted.
