You must be over 18 to register for an account.

I understand that monthly payments are due at the first lesson of each month. I understand that if any amount due to Destination Performing Arts Center is not paid by the 15th of each month, without consulting with office personnel, a $15.00 late fee WILL be applied to my account. I understand that if I allow my tuition to become one month behind without consulting with office personnel my student will not be allowed to return until balance is paid. I also understand that full payment is due each month regardless of my child’s attendance. If I should withdraw my child from Destination Performing Arts Center for any reason, I am responsible for the notification to DPAC and payment until the end of the month during which my child withdraws from classes. I understand that DPAC maintains a No Refund Policy.
***I give permission for my student to be photographed for social media and/or advertising purposes. DPAC will always use discretion when choosing photos.***
DPAC understands that situations happen beyond your control. Please let us know if there is ever a financial hardship that may hinder you from paying on time and we will work with you. Our first and foremost goal is for your student to be the best they can possibly be.

I agree to hold harmless Destination Performing Arts Center from liability from any injury occurring on the premises of Destination Performing Arts Center or any Destination Performing Arts Center event. I give consent for Destination Performing Arts Center staff to render appropriate judgment should my child have an accident or need medical attention and I, nor the emergency contact, are able to be reached. I also understand and agree to alert DPAC in writing immediatlely of any injury my child may have acquired on the premises.
