I agree to abide by all rules and policies set forth online and in the policies form. I agree to obtain and read the studios policy form from Jane's Academy of Dance Excellence and understand that it is my responsibility to do this. I acknowledge that registering with Jane’s Academy of Dance Excellence is an agreement to know and adhere to these policies and have been informed that the studio reserves the right to revise these policies as needed.

I have received or will obtain a copy of Jane's Academy of Dance Excellence policies and will take the responsibility to carefully read and follow the rules and policies therein. I understand that Jane's Academy of Dance Excellence does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc. I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child’s participation with Jane's Academy of Dance Excellence, and I voluntarily assume the risks associated with such participation.

I understand and accept that if I choose auto-draft, monthly tuition will be withdrawn from my account via bank account, credit or debit card on the 1st of each month. I further agree that I am responsible for updating all payment information and that any payment that is denied or declined is subject to an automatic late fee of $25. Tuition is based on a full year and each child is automatically enrolled in classes for the complete 2023-24 season until Jane's Academy of Dance Excellence is notified in writing. I also understand that two weeks written notice of withdrawal is required and that tuition will still be due during those two weeks.

Tuition is due on the 1st of each month. Tuition that is paid after midnight on the 7th of the month will include a $25 late fee and the student will not be allowed to return to class until all tuition and late fees are paid in full.

Electronic Signature Agreement. By selecting the "I Accept" checkbox, I am signing this agreement electronically. I agree this electronic signature is the legal equivalent of my manual signature on this agreement.

I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.

By registering for classes at Jane's Academy of Dance Excellence, you acknowledge that you have read, understood, and agree to the terms of this Privacy Policy.

READ CAREFULLY AND ACCEPT BELOW

I hereby indemnify, release and forever hold harmless Jane's Academy of Dance Excellence, including its owners, employees, assistants, and instructors ( Jane's Academy of Dance Excellence, from any and all claims of liability arising from any accident, personal injury, death, property loss or damage sustained by my child(ren)/myself/the minor child(ren) for whom I am a legal guardian (the Participant) while the Participant is engaged in activities connected with Jane's Academy of Dance Excellence, including classes, rehearsals, performances, or other activities, or is located on any of the premises utilized by Jane's Academy of Dance Excellence before, during, or after such activities.

I understand that dance and performance activities have inherent risks of injury, and, being fully aware of all risk, I consent to have the Participant participate in the classes and activities offered by Jane's Academy of Dance Excellence. I accept full responsibility for providing adequate health and accident insurance coverage for the protection of the Participant. By accepting and consenting to this statement, I declare that the Participant is in good health, with no physical conditions that might prevent participation in strenuous and rigorous dance activities and other training and performance connected with their participation. I understand that I should be aware of the Participant's physical limitations and agree not to exceed them.

Further, I understand and acknowledge that because of the physical nature of dance, there may be physical contact between instructors, assistants, teachers, and students during rehearsals, shows, workshops, productions, and especially during dance instruction. I understand that at times, for proper instruction and safety, physical contact is required and necessary.

Furthermore, I and the Participant agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by Jane's Academy of Dance Excellence.

Further, I authorize and agree that Jane's Academy of Dance Excellence may take and use photographs/videos of me, my invited friends and family, and/or the Participant for purposes of record keeping, advertising, and marketing. I understand that I do not have any rights to these photographs/videos and will not be compensated for the same.

I have carefully read the contents of this waiver form and fully understand its contents. I understand that this is an assumption of risk and release of liability, and I accept it of my own free will.
