Photo Release Form

At Creative Movement Dance Academy, we will take photos throughout the year in which some are used for advertising purposes. I give my consent for Creative Movement Dance Academy to use my child(ren)'s photos, in good taste, for advertising purposes including but not limited to website, Facebook, Instagram, flyers, etc.

Policies - Attendance Policy, Payment Policy, Drop Policy, Facility Policy

Absences: All absences must reported to the studio via your parent portal. Excessive absences are not allowed. If you receive a Dr. Note please send a copy to the studio.
Payments: Tuition is due on the 1st of each month and is non-refundable. If tuition is not paid by the 5th it is considered late. A $10 late fee will be added. Any family who has an overdue bill of 30 days may not be allowed to take part in class. After 90 days past due your account will be subject to collections.
Dropping a Class: Any student wishing to drop a class must have a parent fill out our drop form and turn into the studio. You are financially responsible for all charges. Drop forms must be returned by the 15th of the month to not be charged for the following month. Not attending classes does not drop you automatically from the class. In the event you do not fill out a drop form, you will automatically be responsible for 2 months tuition from when you stop coming to classes. Any accounts 60 days overdue will be subject to collections. No refunds given for tuition, costumes, recital fees, etc.
Facilities:
â€¢ Students will be expected to arrive on time and be ready to go prior to class time.
â€¢ No gum allowed in studio, Water bottles only allowed in classrooms.
â€¢ No jewelry will be allowed that will interfere with dancing. EX. Long necklaces, bracelets, or dangling earrings.
â€¢ Students are expected to always treat their instructor and classmates with respect. The instructor has the right to ask a student who is misbehaving and interrupting class to sit out for the remainder of the class. No refunds will be given for misconduct in class.

Release of Liability Form

As parent/guardian of the child or children I am registering for, I give consent for participation in activities at Creative Movement Dance Academy. I recognize that injuries can occur in this type of activity. I understand that my child must be always be covered by personal health insurance while
participating in activities at Creative Movement Dance Academy. I HEREBY WAIVE, RELEASE, AND DISCHARGE Creative Movement Dance Academy of any and all liability and responsibility for injuries, sickness, pandemics, accidents, natural disasters and/or acts of God incurred
during participation in and/or instruction of camps, intensives, private instruction, choreography or any activity I may participate. I, WAIVE, RELEASE and DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities and or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur. THE FOLLOWING ENTITIES OR PERSONS: Creative Movement Dance Academy and their employees, volunteers or 1099 contractors.
