Consent
As with any physical activity, injuries may occur. Dancing when taught correctly and carefully, will reduce the risk of injury. To reduce the risk of injury I will provide my child or myself with the proper dance attire and shoes required for each class. For the safety of my dancer or myself, I understand that after the first 15 minutes of class, I or my dancer may have to watch and not participate in class after the warm up is over.

In consideration of being accepted as participants in any and all activities and events of the Studio 150 dance program. We (all parties registered and all parents and guardians of minor children) do hereby release Studio 150, The First United Methodist Church of Winter Park, 125 North Interlachen Ave, Winter Park FL 32789, Reeves Center 1100 North Fern Creek Ave, Orlando FL 32803, the Officers of the Organization, Faculty, Directors and Assistants of the program from any and all claims for damages or injuries, which the entrant may sustain while participating in any activity connected with this Program, Competitions and Events. I have also read the Studio 150 Policies and do hereby agree to comply with said policies now in effect or as announced thereafter.

Photo Release
I hereby grant First United Methodist Church Winter Park permission to use my
likeness in a photograph and video in any and all of its publications, including website
entries, without payment or any other consideration. I understand and agree that these
materials will become the property of the First United Methodist Church Winter Park.

I hereby irrevocably authorize First United Methodist Church Winter Park to edit,
alter, copy, exhibit, publish or distribute my photo/likeness for purposes of publicizing the First United Methodist Church Winter Park ministries/programs or for any other lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears. Additionally, I waive
any right to royalties or other compensation arising or related to the use of the
photograph.

I hereby hold harmless and release and forever discharge First United Methodist
Church Winter Park from all claims, demands, and causes of action which I, my heirs,
representatives, executors, administrators, or any other persons acting on my behalf or
on behalf of my estate have or may have by reason of this authorization.
