Tippi Toes® Waiver of Liability (required to take classes):
As a participant in any program or class of Tippi Toes Dance Company® I recognize and acknowledge the nature of the activities my child will be participating in and that my child is in the proper physical condition and capable of participating in the related activities, understanding that Tippi Toes® is not in any way responsible for making such a determination.
I agree to waive and relinquish all claims that I or my minor child may have as a result of participating in any program or class against Tippi Toes Dance Company® and its officers, agents, servants, employees and independent contractors. I further agree to indemnify and hold harmless and defend Tippi Toes Dance Company® and its officers, agents, servants, employees and independent contractors from my claims resulting from injuries including death, damages and losses sustained by me or my minor child that arise out of, in connection with, or in any way associated with the activities of this program.
I understand and agree that by enrolling my child in a Tippi Toes® class located at a daycare, preschool, or elementary school facility, Tippi Toes® has my permission to sign my child out of class and take them to a secure assigned location for dance class. They will be signed back into class or an after-school program at the completion of Tippi Toes® dance class.
Photograph Release:
I hereby permit the use of my, or my daughter/sons image to be photographed, videotaped or otherwise recorded for use in Tippi Toes Dance Company® publicity or educational materials. These materials include, but are not limited to, photographs of classes and performances, newsletters, various other publications, Tippi Toes Dance Company® internet webpage and social media outlets and videos of classes or performances.
I HAVE READ AND FULLY UNDERSTAND THE ABOVE WAIVER AND RELEASE OF ALL CLAIMS.
