Liability Waiver for Next Generation Dance LLC
I have chosen to have my child participate in dance instruction given by Next Generation Dance.
I release Next Generation Dance LLC, it’s instructors, independent contractors, and all other associates from liability for harm, theft, or injury that may be suffered by me and/or members of my family traveling to or from or during participation in activities and programs sponsored by Next Generation Dance LLC. I hereby acknowledge that I am voluntarily assuming full responsibility for all risks of physical injury arising out of active participation in a dance class or other dance related activities.
I acknowledge that the contagious nature of COVID-19 and other contagious diseases and viruses and voluntary assume the risk that I am/or my children may be exposed to or infected by COVID-19 by attending and participating and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 and other contagious diseases and viruses may result from the actions, omissions, or negligence of myself and others, including but not limited to, employees, independent contractors, volunteers, and program participants and their families. I release Next Generation Dance LLC, its instructors, independent contractors, and all associates from liability for harm, injury, or death pertaining to COVID-19 and other contagious diseases and viruses.
I understand there are NO refunds for registration fees, costume fees, recital fees, or classes. I understand and agree to Next Generation Dance LLC policies and procedures. I give Next Generation Dance LLC permission to use my child’s costumes/ accessories until they are no longer required for the season. I understand my student must have this release form signed before attending class. I give permission to Next Generation Dance LLC to use pictures and videos of my child/ children for advertising purposes in print, promotional videos, and on the website.
Tuition is to be paid by automatic withdrawal by the 1st of the month or a $25 late fee will be added. If a payment is 30 days overdue then your child may be dropped from the class. If a payment is not received within a period of 4 months then the account must be handed over to a collection agency. I understand and have read all the studio’s policies and information.

BY REGISTERING AND PAYING FOR ANY CLASS, EVENT, PRIVATE LESSON, OR OTHER ACTIVITY THROUGH THIS WEBSITE, YOU ARE AGREEING TO ALL OF THE ABOVE CONDITIONS AND WAIVERS. IF YOU ARE AN ATTENDEE OF OUR REGULAR CLASSES AND HAVE REGISTERED AND ENROLLED IN A CLASS THROUGH THE PARENT PORTAL AND/OR STUDIO DIRECTOR YOU AGREED THROUGH THERE AS WELL. PLEASE DO NOT HAVE YOUR CHILD/YOURSELF PARTICIPATE IN ANY CLASS OR EVENT UNLESS YOU ACCEPT AND AGREE TO ALL OF THE TERMS IN THIS AGREEMENT.
