Liability Release & Assumption of Risk Form
I/we realize that participation in dance classes and activities could result in some possible personal injury. Despite precautions being taken by Twirl Dance Company, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Twirl Dance Company.
I/we agree to release from responsibility Twirl Dance Company, Chestermere, AB including all teachers, dancers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold Twirl Dance Company, Chestermere, AB, liable for any personal injury including: scrapes, bruises, cuts, sprains, fractures, broken bones, concussions or death or any personal property damage/loss, which may occur on the premises before, during or after classes.
Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by Twirl Dance Company, Chestermere, AB.
I understand that Twirl Dance Company, Chestermere, AB is a licensed and insured organization. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe conduct or conditions to Tarnia Wallace, owner/instructor, as soon as possible.

Photo/ Video Release
Twirl Dance Company and staff may take photos and video of your child to share on our internal Twirl Dance Company Facebook page and shared on our parent portal. Should images be requested for external marketing use, you will be contacted to sign a release form.

Withdrawal & Commitment
We have a happiness guarantee. If your child is demonstrating commitment and effort but is not entirely happy with the class. We will do everything we can to switch them into a more suitable class, if this is not possible then you will be provided with a pro-rated refund less an admin/withdrawal fee of $50 for full-year, and $25 for session classes. Credit is not given for missed classes or sick days.
Applicable for full-year classes up until November 1st and for Session classes, up until the 4th class.
Should a dancer withdraw from the class after costumes have been ordered, it is your responsibility to pay the balance of the costume. In most instances costumes cannot be returned or exchanged.

Illness or Injury
In instances of illness or injury with receipt of a doctor’s note, refunds are given as class credits only.
