I am a parent/guardian of a student that attends DancePro Academy.

Registration
A registration form must be completed. By doing so you are indicating that you agree to comply with the rules and regulations of DancePro Academy.
 Payment
After the initial weekly payment period of approximately 3 weeks, if you wish to withdraw your child from classes anytime subsequently, it must be given in writing to DancePro Academy at half a term. Failure to do so will result in half a term's fees being charged in lieu.
Upon joining DancePro Academy, there is a one off registration fee of £15 which includes a DancePro Academy T-shirt that can be worn in all classes except ballet.
Fees for the following term are strictly paid before the end of the current term or a 10% increase will incur
Fees are non-refundable once paid.
The preferred method of payment is by electronic bank transfer however cheques payable to ‘DancePro Academy’ and cash is also accepted.  We regret that we do not accept credit cards.
 Classes
Pupils are required to wear the correct uniform as this provides a disciplined approach to class.
Hair should be worn neatly and preferably in a classical style for ballet.
Please refrain from wearing jewellery to class.
DancePro Academy is not responsible for the loss of items. 
Please inform the teacher of any injuries, illness or medical conditions.
 Liability 
Pupils are obliged to take care of their own belongings.  The Academy cannot accept responsibility for lost or damaged items
Parents must take full responsibility for children aged 16 and under whilst they are attending DancePro Academy classes.
The liability of DancePro Academy and its staff is restricted to class time only.
 Other
DancePro Academy reserves the right to make changes to the current timetable and to the teaching staff. 
Please inform DancePro Academy if you DO NOT wish your child to be in photographs or films taken during classes and demonstrations. Any media collected will be for advertising purposes only.  ie: Website, social media, newspapers etc
When classes are cancelled they will be made up during the next available school holiday.
Please notify a member of staff should pupils be unable to attend class.
No food, chewing gum or fizzy drinks to be consumed in the studio or during classes.  Only water, squash or fruit juice. 
Pupils and parents must be respectful of each other and to all staff members. 
DancePro Academy has the right to remove pupils from classes should the terms and conditions fail to be met.
I have read and understood the terms and conditions set out by DancePro Academy and agreed to them.

Either Party shall be excused from performance and shall not be in default in respect of any obligation hereunder to the extent that the failure to perform such obligation is due to a Natural Force Majeure Event.

Parent Release Form for Photography and Videography
DancePro Academy may use video footage and / or photographs of students. This usage may include (but is not exclusive to) displaying publicly, distributing, or publishing, photographs, and/or video of my child for use in materials that include, but may not be limited to:

 - printed materials (eg - brochures and newsletters)
 - online and offline advertising and promotion
 - videos and digital images such for use on Social Media. Images may be used by DancePro Academy without further notification.

Health Information and Medical Release/Waiver Form

I acknowledge that participation in dance is potentially dangerous and there is an inherent risk of injury involved. In allowing my child to participate in DancePro Academy’s activities, I hereby assume all the risks associated with the performing arts. I understand the importance of myself and my child following the instructions and rules set by their instructor/s, and I agree to release DancePro Academy and it’s employees of any and all liability which may arise as a result of my child’s participation in activities at DancePro Academy

Notice Period contract
I acknowledge and understand I must give half a term’s notice, to withdraw my child from their class, in writing. Failure to give notice will result in half a term’s fees being charged. I am aware that late payments incur a 10% surcharge
