I am fully aware of and appreciate the risks, including the risk of catastrophic injury, paralysis and even death associated with participation in dance and gymnastics activities. I have communicated these to my child(ren). I further agree that Art of Dance and its staff shall not be held liable for any losses or damages occurring as a result of my child(ren’s) participation. I have read and agree to comply with Art of Dance’s policies.

I agree to paying the amount listed for tuition each month. I understand that a late fee of $15 will be applied to my account if the monthly tuition isn't paid by the 15th of each month. I understand that the tuition amount is a yearly tuition broken down into monthly payments, that being said, regardless of holidays or missed classes during the year, the monthly tuition amount will remain the same. You are welcome to come to make up classes for missed classes. I understand that if my account goes to collections, I am responsible for attorney's fees and all other expenses at that time.
