I ALSO UNDERSTAND THAT I WILL GIVE A THIRTY DAY NOTICE IN WRITING IF MY CHILD DECIDES TO NOT TAKE CLASS AT CEDARTOWN ACADEMY OF PERFORMING ARTS. OTHERWISE, WITHOUT THE THIRTY DAY WRITTEN NOTICE, I WILL BE CHARGED TUITION UNTIL I SEND THE WRITTEN CONSENT AND THE TUITION WILL BE STOPPED AFTER THE THIRTY DAYS.

I will not hold the Cedartown Academy of Performing Arts nor its staff and owner for any liability, accidents, lost items responsible at any time for class, performances, competitions and workshops.

I DO UNDERSTAND THAT IF I WISH TO WITHDRAW MY CHILD/CHILDREN FROM CLASS OR CLASSES AT THE CEDARTOWN ACADEMY OF PERFORMING ARTS, I MUST GIVE A 30 DAY ADVANCE NOTICE IN WRITING TO OFFICIALLY WITHDRAW FROM CLASS OR CLASSES. I DO UNDERSTAND IF I DO NOT GIVE THE 30 DAY WRITTEN NOTICE OF WITHDRAWAL, I WILL BE CHARGED UNTIL I GIVE THAT NOTICE.

TUITION IS DUE BY THE FIFTH OF EACH MONTH. IF TUITION IS NOT RECEIVED BY THE 5th OF THE MONTH A $5.00 LATE FEE WILL BE ADDED EACH WEEK UNTIL TUITION IS RECEIVED.

ANY TUITION TWO MONTHS BEHIND WILL AFFECT STUDENTS PARTICIPATION FOR CLASSES AND PERFORMANCES. ALL TUITION MUST BE UP TO DATE AND PAID ON TIME.

Tuition is not based on single lessons or the number of classes per month, but it as a YEARLY fee for the total classes, workshops and rehearsals provided in THE SCHOOL TERM. For your convenience, payments may be made by the month. Annual payment is also accepted.

*Monthly TUITION payments are offered as a courtesy to our families, but do not reflect an even number of lessons per month. * Allowance is made for holidays, and a discount is already given for students taking more than one class per week. We regret we do not accept DEBIT OR credit cards of any kind "EXCEPT ONLINE" THROUGH CASHAPP or Venmo --cash,check or money orders are accepted in class, but we DO encourage you to pay online through the CASHAPP or Venmo account. Fees are due and payable in advance in the following method:
*Monthly payments: Due by the 5th of each month. Late fee assesses after the 5th.
*Family Discounts: IMMEDIATE FAMILY SIBLINGS ONLY AND LIVING AT THE SAME RESIDENCE.

With this system you will be emailed a monthly statement, however, tuition is due by the fifth of each month. A $5.00 surcharge or 10% of the total due, whichever is greater, will be assessed after the 5th of the month. A $36.00 service fee is charged for all returned checks. There will be no refunds. No refunds are given on tuition paid in advance or for classes missed due to the student’s absence. NO REFUNDS ON DANCE COSTUMES!
