I hereby give to Genese Brisky, or anyone she my designate, authorization to contact any medical and/or emergency assistance she may deem necessary.

I understand that Partners Dance Studio LLC and/or Genese Brisky will not be responsible for any medical payments or fees charged as a result of any accident or injury or the emergency treatment thereof.

I hereby release photos and video footage for Partners Dance Studio LLC publicity.

The photographs and videos are intended for marketing and promotional use on our website or social media. These photos do not name or disclose personal information of those within the images.
