I have received and read the policies of Kristin Butke Irish Dance. I recognize that regular class attendance is important for student progress and that it is my responsibility to attend all classes. I also understand that failure to attend the lessons does not relieve me of my obligation to pay tuition; four weeks written notice will be required for withdrawal from class(es) for which I have registered. If I do not submit written notice and I stop attending class, I will be liable for the remainder of the session tuition. I also understand that all tuition and fees are non-refundable and non-transferable.

While Kristin Butke Irish Dance is committed to conducting its programs and activities with the safety and well-being of its students in mind, I recognize that participation in any of these programs or activities involves some risk of injury. I certify that I am able to participate in any program or activity with Kristin Butke Irish Dance. I further understand and agree on behalf of myself, my heirs and assigns, that Kristin Butke Irish Dance, its directors, officers, staff, instructors, volunteers, landlords or lessors and VIDA, in whole or in part, are not responsible for any and all illness, injury, or damage that I may suffer as a result of my participation in any of Kristin Butke Irish Dance classes, rehearsals, performances, and related programs or events.

I understand that any/all photos taken of me/my child as a participant in any school classes/events may be used for publication and/or team promotion. I give Kristin Butke Irish Dance permission for my/my child's photo to be used for school promotion. I understand that NO last names/personal information will be used in conjunction with any photos.

I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.

By selecting the "I Accept" checkbox, I am signing this agreement electronically. I agree this electronic signature is the legal equivalent of my manual signature on this agreement.

Safety

While Kristin Butke Irish Dance is committed to conducting its programs and activities with the safety and well-being of its students in mind. I, on my own behalf and on behalf of Minor, realize and are fully aware that participation in dance classes and related activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I (the dancer/parent/guardian) assume all risks related to the use of any and all spaces used by Kristin Butke Irish Dance.

I agree to release and hold harmless Kristin Butke Irish Dance, including its teachers, dancers, staff members, and facilities from any cause of action, claims or demands now and in the future. I will not hold Kristin Butke Irish Dance liable for any personal injury or any personal property damage suffered by me, my child or family member which may occur on the premises before, during or after classes and/or while participating in any Kristin Butke Irish Dance sponsored event whether or not held at the premises. Furthermore, I agree to obey the class and facility rules and take full responsibility for my behavior in addition to any damage I or family members may cause to the facilities utilized by Kristin Butke Irish Dance.

Loss or Theft

I agree to hold harmless Kristin Butke Irish Dance for loss or theft of personal articles suffered by me or family members while participating in activities at Kristin Butke Irish Dance and/or attending a related event.


Payment Policies

We accept all forms of payment to include cash, check, debit, credit, and auto-pay. Auto payment (preferred method of payment) is available for those who wish to avoid late fees. Full tuition is due on the 1st of the month. A late fee of 10% is charged for tuition payments not received online or in the office on or before the 5th of each month. A $30.00 returned check fee will be charged for all returned checks. As of July 1, 2023, any/all bank fees incurred for a returned check will also be billed to the client. Any account past due more than 30 days will be removed from our program. Delinquent accounts are subject to being forwarded to our collection agency and subject to any applicable fees. Withdrawal

*Drop in class students are excluded from withdraw notice stated above*

Injury Policy

Should a period of classes 1 month or more be missed due to injury or illness, full tuition amount will not be charged. A doctor's note must be submitted when informing KBID of injury or illness. Upon receipt of doctor’s note, a $60 monthly tuition will be charged for prizewinner and up and $30 monthly tuition will be charged for novice and under. This will retain a dancer's place in class and give the dancer the opportunity to be present in class and continue to learn and take notes (illness must not be contagious). These rates will be prorated, based on the week of return, should a dancer return within a billing cycle. If injury tuition is not received, the family will be subject to re-enrollment and charged a registration fee upon returning to class.

*Please note: if you paid the semester in full, there will be no refunds for any reason, including dancer's injury.*

Photo/Video

I hereby consent to and authorize the use and reproduction by Kristin Butke Irish Dance of any and all photography and/or videography that has been taken of me/or my family for the purpose or marketing including but not limited to website, brochures and social networks. Names will not be used unless it is a private group within Kristin Butke Irish Dance only.

Infectious Diseases

I am aware that possible exposure to infectious diseases, including, but not limited to, MRSA, Influenza and COVID-19 are a potential risk by participating in class(es) or other activities with Kristin Butke Irish Dance. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist. I agree to hold harmless Kristin Butke Irish Dance for loss or suffering by me or family members due to participating in activities/classes at Kristin Butke Irish Dance.

**All dancers and their families are expected to follow all Kristin Butke Irish Dance policies, whether written or verbal. KBID has the right to dismiss dancer(s) and their families at any time for not adhering to the policies.**
