1. General Waiver for remote/virtual classes as applicable: I agree that Dance Magic, its dance director Karen Dycaico, its teachers and staff members, Cubberley Community Center, and the City of Palo Alto are not liable for injuries that may occur during the course of instruction online.

2. General Waiver for in-person classes as applicable: I agree that Dance Magic, its dance director Karen Dycaico, its teachers and staff members are not liable for injuries that may occur during the course of instruction at the home or studio.

3. For In-Person Classes only---Waiver of Liability for COVID-19: Dance Magic, its director Karen Dycaico, its teachers and staff members have put in place preventive measures to reduce the spread of COVID-19. I acknowledge the contagious nature of COVID-19 and will not hold liable Dance Magic, its dance director Karen Dycaico, its teachers and staff members for any medical issues that may result from COVID-19 exposure during the course of the dance class at the home or studio.

4. Photos and Videos taken during classes and shows may be used for marketing purposes.

Health and Safety Measures:
- You must be healthy and well to attend class, please make sure your child does not have a fever, cough or cold. Notify me if anyone gets sick.
- All teachers and dance students are advised to wear a mask during class time and class interaction
- Students are recommended to sanitize hands before and after class. Hand sanitizers will be available in the studio’s entrance.
- Windows will be open for ventilation and air purifiers will be operating at all times
- Only students/teachers will be allowed inside the studio, parents/guardians will have to wait outside
