WAIVER
Prior to your dancers first class, you are required to read and check the waiver in the Parent Portal. To access the waiver, sign into your parent portal, and at the top of the page click MORE>WAIVER. By checking the box, you agree to the following:
"I do hereby fully release and discharge Master Dance Academy, its officers, agents, servants and employees from any and all claims from injuries, damage or loss which I (or dependants under 18 years of age) may have or which may accrue to me (or dependants) on account of my (their) participation in the above Master Dance Academy sponsored activities. I further agree to indemnify and defend Master Dance Academy, its officers, agents, servants and employees from any and all claims resulting from injuries, damages and losses sustained by me (or dependants under 18 years of age) and arising out of, connected with, or in any way associated with these activities sponsored by Master Dance Academy. I grant permission to use my (child's) picture/image on the website and/or advertising for Master Dance Academy. I have read the "Policies" and agree to adhere by the procedures mentioned. Instructors are subject to change without notice and reserve the right to place student in correct level. I understand the cancellation policy particular to the program my child is participating in."
PROMISE OF PAYMENT.
Parent agrees to pay all tuition, competition, costume and other related fees by the date set by MDA and understands that nonpayment will result in the dancer being removed from classes/ choreography. MDA agrees to give the parent/dancer at least two weeks verbal and one week written notice of payment due dates.
Parents agree to help the dancer meet the commitment they have made by signing the Code of
Conduct agreement.
It is mandatory that one Parent / Guardian attend three (3) informational meetings during the year. One on December 16, 2017, another in January and the final in May/June (dates to be announced upon returning in the New Year)
Policy and Procedure
1.Three warnings will be given to students who have unexcused absences from class, excessive tardiness and/or are unprepared for class. After three warnings, student will be removed from competitive classes. The warnings will be 1st to the student from the Teacher and or Artistic Director,
2nd to the students and their parents from the Teacher and or Artistic Director and 3rd in writing from the Artistic Director and The Executive.
3.Students not in attendance at regular classes as of November 1 will not be allowed to compete with the class.
Costume and Competition fees are non refundable after November 30.
Notify the studio and inform the teacher [if possible] when my child will be absent from class
Ensure to bring my child to class on time to avoid disrupting the teacher and other students. [Warm-up is conducted at the start of every class and it is imperative that all dancers participate in warm-up to ensure that their body is ready for physical activity.]
Ensure my child is dressed in the appropriate clothing and shoes to each class in accordance with the uniform requirements as listed in the Parent Handbook, including having hair in the appropriate style and removing all jewelry before each class.
Focus on my child’s performance rather than the overall outcome of the examination, performance or audition.
Encourage my child to follow the rules of the dance studio and any competition or audition they enter into. Notify the office of any change of Contact information.
Be financially responsible for any damage that my child causes to studio property.
Conduct myself with behaviour that is beyond reproach at Community and Festival events.
Support all efforts to remove verbal and physical abuse from all dance activities. Applaud performances by all of the performers, not just my own child.
Respect and follow advice and direction provided by the Dance Teachers.
Abide and respect all Policies and Procedures set forth by MDA
Inform MDA in writing of any extended vacations or absences, which will affect my child/children. I will not:
Make disparaging remarks of any type with regard to teachers, choreographers, board or staff members, choreography placement, costumes, other parents and dancers, other schools or publicly criticize them in the studio or at any event.
Ridicule or yell at my child, or any other child, for making a mistake or not performing to my expectations.
Upload any MDA dances or choreography onto Facebook, YouTube, or other similar sites as this could breach copyright.
Smoke anywhere in the studio or immediately outside the studio. Park or stop in the hand cap parking outside the front door of the studio.
Raise an issue with or question a teacher’s judgment in public; instead raising any issue or disagreement in writing to the Artistic Director or the MDA Board.
I understand that:
If my child exhibits inappropriate behaviour he/she will be removed from the dance class. I agree to partner with MDA staff to resolve behavioural issues immediately so my child may return to class promptly.
The final decision regarding my child’s ability to compete in any discipline will be made by their teacher & the Artistic Director, Monica.
Children who are not dancing may not be left unsupervised at the studio.
Students must be picked up within 15 minutes from end of class time.
MDA reserves the right to limit photography or videotaping at any MDA shows.
That there will be NO refunds for missed classes.
That if my child is removed from competition for any reason I am still responsible for all costume and Competition fees.
It is my responsibility to make sure I am receiving all newsletters and information given out at the studio and via email.
Parents/Guardians must supervise their children while waiting at the studio to ensure that they do not disrupt classes, injure themselves or other dancers, or cause additional mess and havoc. Parents/Guardians accept full responsibility for damages created by unsupervised children;
Any abuse, verbal or otherwise, perpetrated by a member of the studio towards any other member, teacher, or staff will result in immediate removal from the studio.
I hereby authorize Master Dance Academy to charge my credit card for tuition or other charges (performance fees, costumes, registration, etc.) incurred by my family’s participation in classes at the Academy. If classes are added after registration, the amount will be adjusted accordingly. I understand that the debit/credit card transaction will occur on the 23rd of the prior month for all enrolled classes. I understand that all drops or withdrawals must be made in writing prior to that time. Families are responsible for all class fees regardless of attendance.
Tuition is due on the 28th of each month for the following month. Credit/Debit Cards will be charged at that time. It is your responsibility to keep your credit/bank card number and expiration date current. We will attempt to contact you of any problems, but late fees will incur if we are unable to process your card by the 8th of each month.
