I hereby release and hold harmless In the Wings Academy of Dance, its director, teachers, employees, and staff from and against any liability or claim for any loss of property, injury, misadventure, harm, cost, or damage sustained by me and/or my child as a result of my child’s participation in classes, performances, and activities of In the Wings Academy of Dance.
I acknowledge the contagious nature of COVID-19 and voluntary assume the risk that I and/or my children may be exposed to or infected by COVID-19 by attending and participating and that such exposure of infection may result in personal injury, illness, and permanent disability.
I understand that the risk of being exposed to or infected by COVID-19 and other contagious diseases and viruses may result from the actions, omissions or negligence contractors, volunteers, and program participants and their families.
I release In the Wings Academy of Dance LLC, it’s instructors, independent contractors and all associates from liability for harm, injury or death pertaining to COVID-19 and other contagious diseases and viruses.
I understand and agree to In the Wings Academy of Dance LLC policies and procedures and agree to all rules and regulations.
I understand that my student must have this release form signed prior to attending classes.
I give my consent for images (photographs, video) of my dependent to be taken and used to document the activities of In the Wings Academy of Dance. I grant In the Wings Academy of Dance permission to use the images for educational and promotional purposes. I understand that if I do not want images of my dependent to be used, I will indicate this in writing.
