I understand that there are risks of physical injury or illness associated with, arising out of and inherent to the activity of dance. In recognition of this acknowledged risk of injury or illness, I knowingly and voluntarily waive all right and/ or causes of action of any kind including any and all claims of negligence arising as a result of such activity from which liability could accrue to That's Entertainment Performing Arts, its officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities (hereinafter collectively referred to as That's Entertainment Performing Arts).
I hereby agree to release That's Entertainment Performing Arts and hold That's Entertainment Performing Arts harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury or illness arising out of active participation in dance on behalf of the participant. I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury or illness. I have agreed to these terms voluntarily and of my own free will in exchange for the privilege of participation. I authorize That's Entertainment Performing Arts to seek medical attention for my child via an ambulance or emergency room visit if an emergency medical situation arises before, during, or after class. If I am a minor, my parent and / or legal guardian has agreed to these terms releasing That's Entertainment Performing Arts from any and all such liability described above and has acknowledged that I am knowingly and voluntarily assuming all risks of injury inherent to this activity.
I authorize That's Entertainment Performing Arts to use my/my child's picture or videos on any form of advertisement, for rehearsal purposes, or other internal or external uses to promote or enhance classes for That's Entertainment Performing Arts or a That's Entertainment Performing Arts affiliated event.
I have read, and agree to, the full list of studio policies located in the Studio Handbook.
