*I for myself and or minor child release and forever discharge and hold harmless Studio Geaux and any of its instructors for any liability, claims or demands for personal injuries, sickness (any Covid-19 related illness) as well as expenses due to loss or damage to personal property. Since dance and tumbling are physical activities, injuries may occur. Each student registers and enters into activities in the studio by choice. Parent or guardian is fully responsible of informing instructors of any physical limitation that may prevent full participation in class. Parent/Guardian must sign below in agreement with all terms of liability disclaimer and studio policies.

*By signing below, adult of guardian assumes full financial responsibility for all tuition and fees. If for any reason dancer decides to no longer be a student at Studio Geaux, he or she must present instructor with a written notice and will be responsible for payment of any classes attended. Additionally, said person agrees to read and abide by all studio rules and payment policies. There are no refunds for registration fees, tuition, costumes, apparel, private lessons, or recital.
