You must be over 18 to register for an account.

By signing this agreement, you agree/consent to all fees set forth by Jamie's Performing Arts Studio and the established payment due dates. Jamie's Performing Arts Studio fees may/will include registration, camps, clinics, monthly tuition, recital, music, costumes (includes but not limited to dance shoes, tights, jewelry, hair pieces, etc.), competitions (includes but not limited to registration, entry, convention, administrative, etc.), private lessons, parades, street fair, community events, and any other event that Jamie's Performing Arts Studio chooses to participate. You also agree to pay any late fees incurred due to late/non-payment of the above-mentioned items. All Items are non-refundable and non-transferable.

Student tuition/private lessons must be paid on or before the first of every month. Cash or check may be paid on or before the first of the month. Autopay will be run on the second of the month. All other fees must be paid by the due date set by Jamie's Performing Arts Studio. If fees are not paid by the due date, a $25 late fee will be assessed to every late item per month.

If any student chooses to leave Jamie's Performing Arts Studio, a 30-day written notice is required in order to end the accumulation of fees. All fees accrued to that date are still payable to Jamie's Performing Arts Studio and due within 15 days from end date. You will be notified of your balance due, if any. Failure to pay the balance due within the 15-day grace period, Jamie's Performing Arts Studio will be forced to take legal action against you for the balance due and you agree to pay all legal fees incurred by Jamie's Performing Arts Studio.

In the event of legal actions taken, the account holder will be responsible for accrued fees from the time of entrance into the start of the dance season thru the ending of the calendar dance year ending at the recital. The account holder will also be responsible for additional late fees and a daily fee of $5.00 per day for all fees accumulated after agreed deadlines.

WAIVER RELEASE FORM
RELEASE FROM LIABILITY, WAIVER OF CLAIMS & ASSUMPTION OF RISK by signing this Release, Waiver & Assumption of Risk, (the 'Release') I agree that Jamie’s Performing Arts Studio shall not be responsible for any injury suffered by the student. The Release extends to Jamie’s Performing Arts Studio, and its owners, officers, agents, and employees (all of which are collectively referred to herein as 'studio'). The Release extends to every claim, demand or liability of any kind based on any injury or damage described below.
I understand that dance and music activities, as conducted and taught at the studio, have inherent risks of injury. These risks include muscle pain and pulls, broken bones, ankle injuries, and other personal injury. I recognize that the student is exposing himself/herself to such risks when undertaking dance activities. I understand that these risks cannot be fully eliminated without jeopardizing the essential qualities of the activity. The student and I assume and accept all risks of injury or damages resulting from such dance activities. The student's participation in this activity is purely voluntary, and the student elects to participate, and I join in that election, despite the risks.
My Child/Children and I agree to observe and obey all posted rules and warnings, and further agree to follow any oral instructions or directions given by Jamie’s Performing Arts Studio Instructors, owners, or representatives of the studio. I agree to pay for all damages to the facilities of Jamie’s Performing Arts Studio cause by my child or family’s negligent, reckless, or willful actions.
Jamie’s Performing Arts Studio publishes a Facebook page, in which it posts photos and videos of dancers and music students. A student’s photo could be published as part of a performance, group shot, events, solo picture, competition, or award recognition. If you do not wish to have your dancer’s photo posted on the website, newspapers, internet, or social media, please contact us in writing stating you do not wish to have photos published on the internet. By signing this waiver, you are giving Jamie’s Performing Arts Studio permission to publish photos/videos on the internet.

I also agree that the studio, including its owners, officials, agents, instructors and employees, are not responsible for any property loss or damage suffered by the student or any guests or parents of the student that results upon entry into or presence in the studio or its surrounding property. I further understand that I have been advised of the need for the student to be covered by adequate insurance to cover any injury or damage that may be suffered while participating, and I have obtained such insurance or have agreed to bear the costs of any such injury or damage myself.

I have read and understood the 2024-2025 year policy and procedure packet. I agree to the dress code and attendance policy. I further agree to the billing policy that will be strictly enforced. I fully understand my child/I WILL NOT be able to attend any classes if my account is delinquent. I also understand that once my account is paid up to date, the student will be allowed to return to classes. I further understand that I have to pay tuition on or before the 1st. If not paid on the first, I understand autopay will be run on the second. I also understand that any other fees or items will be assessed a late fee of $25.00 if not paid on or before due date.

Emails will be sent periodically with important information and updates.

Jamie’s Performing Arts Studio will not accept responsibility or liability for injuries sustained by anyone including dancers at our studio or during any traveling events. I also understand that I am in a studio contract from August until the yearly showcase. I understand that if I break that contract then costumes, shoes and all Jamie’s Performing Arts Studio related materials and items are in the studios possession if not received.
