RELEASE AND WAIVER
Each student or parent/guardian of a student must complete, sign, and date this form before participating in any class, workshop or performance. Please sign and date this Release and Waiver, then return it with your registration form.
RELEASE AND WAIVER I, the undersigned, do hereby release, waive, discharge, indemnify and hold harmless Dance Arts by Regina, LLC (the “Studioâ€) and their affiliates, owners, members, directors, officers, employees and agents (“Releaseesâ€) from any and all liability for injury, loss, damage, obligation, expense or penalty, including attorneys’ fees (the “Lossesâ€), that I, or my student, may cause or sustain in connection with my, or my student’s, participation with the Studio’s classes, workshops or performances, whether such Losses occur on the Studio’s premises or elsewhere and whether such Losses are caused by the negligence or carelessness of Releasees or otherwise. I understand that Releasees do not have medical personnel available at the Studio and agree that Releasees assume no responsibility for any injury or damage that might arise out of or in connection with my, or my student’s, participation with any Studio activity (whether on the Studio’s premises or elsewhere).
By signing below, I, or my student, knowingly and voluntarily assume(s) all such risks, and assume(s) sole responsibility for any accident or injury to me/him/herself and other persons or property arising out of or in connection with participation in Studio classes, workshops or performances (whether on the Studio’s premises or elsewhere). In addition, I represent that I maintain (or am covered by) a health and accident insurance policy that provides adequate coverage for the activities in which I, or my student, may engage.
I authorize the Studio to use my, or my student’s, name and likeness for commercial or promotional purposes without further approval, permission or consideration. I further agree that any images or videos taken of me or my student during any Studio class, workshop, performance, program or event may be used by the Studio for archival, documentation, commercial or promotional purposes, including on the Studio’s social media platforms.
In signing this Release and Waiver, I represent that I have read its contents, that I understand it and that I sign it voluntarily or on my student’s behalf as my own free act and deed. If applicable, I represent that I have full legal authority as my student’s legal guardian to enter into this Release and Waiver. I further state that I am at least eighteen (18) years of age, am fully competent to sign this document and that I execute this Release in consideration of my or my student’s participation in the Studio’s activities and programs, fully intending to be bound by same. This Release and Waiver will be governed by the state law of Ohio.
