Student and parent/ guardian agree to the following:
Tempo Dance and its employees are not liable for injuries or illnesses contracted by the students while attending the studio. Tempo Dance and its employees are not liable for any damaged, lost or stolen items.
In signing this waiver I am agreeing to make payments for the season. If I decide to withdraw from a class I must do so in writing and will be charged for the current month and will be responsible for any past fees which includes but is not limited to tuition, competition fees, costumes, accessories, shoes and any other items currently on account or that I have already been registered for. No refunds on shoes, competition fees, costumes, registration fees or tuition.

I hereby authorize and give my full consent to Tempo Dance Studio to copyright and/or publish any and all photographs, videotapes, films web casts/podcasts and any other form of visual/audio communications in which I appear while attending a Tempo Dance Studio events including but not limited to classes, recitals, performances, events and competitions and conventions. I further agree that Tempo Dance Studio may transfer, use or caused to be used, these above mentioned communications for any exhibitions, public displays, publications, commercials, art and advertising purposes, social media and television programs without limitations or reservations.

By Signing this release, Waiver and Assumption of Risk, (the "Release"), I Agree that Tempo Dance Studio shall not be responsible for any injury suffered by the student ("student"). The Release extends to Tempo Dance Studios, its owners, officers, instructors, employees and locations of classes (all of which are collectively referred to herein as the "studio"). The Release extends to every claim, demand or liability of any kind based on any injury or damage described below.

I understand that dance activities as conducted and taught at the studio have inherent risks of injury. These risks include but are not limited to, muscle pain and pulls, broken bones, ankle, knee and back injuries and other personal injury. I recognize that the student is exposing himself/herself to such risks when undertaking dance activities. I understand that these risks cannot be fully eliminated without jeopardizing the essential qualities of the activity. The students and I assume and accept all risks of injury or damage resulting from such dance activities. The student's participation in this activity is purely voluntary, and the student elects to participate, and I join that election, in spite of the risks. I also agree that the studio, including its owners, officers, instructors, employees and locations of classes is not responsible for any property loss or damage suffered by the student, or any guests or parents of the student, which results upon entry into or presence in the studio or its surrounding property.
I further understand that I have been advised of the need for the student to be covered by adequate insurance to cover any injury or damage that may be suffered while participating and I have obtained such insurance or have agreed to bear the costs of any such injury or damage myself.
I understand the terms of this Release and have had the opportunity to consider and discuss it with such individuals and advisors as I deem appropriate. By signing and dating this Release, I confirm that I have read the Release in full, that I understand its terms, and that I agree with those terms. I further confirm that by signing this Release, I acknowledge that if the student is hurt during participation in this activity, I may be found in a court of law to have waived my right and any right of the student to maintain a lawsuit against the studio on the basis of any claim which is released hereby. In consideration of the students’ being permitted by the studio to participate in the activities provided by the studio, I further agree to hold harmless and indemnify the studio for any and all claims which are brought by or on behalf of any student who is a minor and which are in any way connected with the activities performed at the studio by any such minor student.

Tuition is due on the first of the month. Late fees will be added to tuition on the 6th of the month. Classes and fees are non-refundable. No refunds on registration fees, tuition, costume fees, accessories, competition fees, recital tickets. If withdrawing from a class you will need to notify the studio one month in advance to avoid charges for the following month.
