I hereby release Accent School of Dance, Karen Rich, and all staff members of any, and all responsibility for personal injury and/or accidents which may occur while participating in any classes, workshops or programs. I understand that I am enrolling my child in a program of physical activity and have agreed that my student is in good physical condition. In the case of emergency, I grant permission to the staff of Accent School of Dance to take first aid or emergency measures as judged necessary for the care and protection of my child while under the supervision of the studio.

I herby agree to abide by the price structure. I understand the late fee policy and agree that any past due accounts will be responsible for late fees. I understand no further discount or refund will be made for missed classes.

I acknowledge, and agree, that my Participant's participation in any and/or all events and activities involves known, unknown,anticipated and /or unanticipated risks that may result in:

illnesses, infections and /or viruses including but not limited to Corona,Covid,Ebola,Mers and /or Sars caused by but not limited to bacteria, virus, and or fungi.

I agree my dancer is in good health and will not hold Accent School of Dance, Karen Rich and all staff members liable if my dancer becomes sick.
