By completing this registration, I acknowledge and agree to each of the following:
1. Enrolling in a dance class is an 8-month commitment.
2. Tuition payments are due on the 15th of each month and are considered delinquent if not paid by the 20th of each month. Delinquent amounts will accrue late fees and interest as set out in the Dancer’s Pointe Handbook.
3. Tuition payments and all other fees (costume fee, performance fee, rehearsal fee, etc.) must be current in order for student to participate in any Dancer’s Pointe performance or scheduled activity. I understand that additional fees will be owed in the event my student signs up for opportunities or activities in addition to regularly-scheduled classes (e.g., workshops, private lessons, special performances).
4. If my student needs to withdraw from a class, I am responsible for tuition (and any additional fees) in full until my written notice of withdrawal has been received by the Dancer’s Pointe office. Not attending class does not constitute withdrawing. A parent or student verbally telling an instructor that the student will no longer attend class does not constitute withdrawing. I understand that tuition and fees will continue to accrue until I withdraw the student using a written withdrawal notice (please refer to the withdrawal policy in the handbook). I understand and agree that tuition and fees are not refundable for any reason.
5. I agree to pay Performance/Costume Fees for all Dancer’s Pointe performances by the installment dates on which they are posted as due.I understand that, once the first installment is paid, these fees are not refundable even if the student withdraws from the school and/or performance. If the office does not receive written notice pursuant to Paragraph 6 that a student will not participate in the Nutcracker or year-end performance, a costume will be ordered for each student who is enrolled in a class. (All ballet students participate in the Nutcracker and all students participate in the year-end program. Performance fees are charged and costumes are ordered for both performances. In addition, ballet students enrolled in the Academy participate in a year-end Showcase and Honors Ceremony, for which additional fees may be charged.) I understand that, if my child’s costume does not arrive in time for a Performance due to my failure to pay an installment or my child’s absences, my child will not be permitted to perform.
6. I agree to notify the school office, in writing, no later than Sept. 1, if my child is NOT participating in the Nutcracker and no later than Feb. 1 if my child is NOT participating in the year-end program. If notice is not received, I am responsible for the costume fee even if my student does not participate in the show. In that event, I understand that I can pick up the costume after the production but no later than January 10th (for the Nutcracker) and June 1 (for the year-end program).
7. I agree to sign up for at least one volunteer slot for either Nutcracker or the Spring Recital. If I do not wish to volunteer, I agree to pay a $50 fee at the time of enrollment.
7. I understand that Dancer’s Pointe reserves the right to cancel any class that does not have a sufficient number of students enrolled to support the class.
8. I agree that my child and I will adhere to all rules and requirements stated in the Dancer’s Pointe Handbook, including the dress and hair code. In addition, if my child participates in any Academy classes, I agree that my child and I will adhere to all rules and requirements stated in the Academy Handbook, including but not limited to the attendance policy. I understand that these documents are available on my Dancer’s Pointe portal.
While participating in Dancer’s Pointe activities, students are representatives of Dancer’s Pointe and are expected to adhere to the etiquette expectations and discipline policy contained in the Handbook. Dancer’s Pointe reserves the right to immediately and without notice dismiss any student whose behavior or conduct is unacceptable and/or disruptive, as determined in the sole discretion of the Director.
9. I acknowledge and agree that photographs, videos, or audio recordings of my student may be taken during classes, performances, and other Dancer’s Pointe activities, and I authorize and grant permission for the use of such photographs and recordings and of my child’s name and image for inclusion in publications, websites, and authorized social media of Dancer’s Pointe.
10. This Registration Agreement constitutes as a contract between myself and Dancer’s Pointe. I understand and agree that, in the event of any breach of this Agreement, Dancer’s Pointe shall be entitled to recover its attorney’s fees and costs.
11. I understand that dance is a physical activity with an inherent risk of injury and, in exceedingly rare cases, death. By enrolling my child, I assume all risks related to my child’s participation in Dancer’s Pointe classes, productions, and activities. I further certify that my child is physically fit, and has not been advised to avoid dance class or other physical activities by any qualified medical professional. I certify that there are no health-related reasons or problems that would preclude my child’s participation in Dancer’s Pointe classes, productions, and activities.
In consideration of my student’s opportunity to enroll, and on behalf of my student and myself, I hereby release, discharge, waive, indemnify (including, without limitation, attorney’s fees and costs), and hold harmless Dancer’s Pointe, LLC, its employees, agents, volunteers, and representatives for any injuries, death, illness, disability, pandemics, damages, theft, losses, or claims, whether known or unknown, that may arise directly or indirectly from my student’s participation in Dancer’s Pointe classes, productions, or other activities, whether such claims are brought by me or by anyone else acting on my behalf. I also acknowledge and agree that Dancer’s Pointe, LLC, and its employees, agents, volunteers, and representatives are not responsible for and shall have no liability with regard to my student’s personal belongings or personal property. This waiver and release shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.
I understand that, in the event medical intervention is needed, every attempt will be made to contact the person(s) listed on the student’s registration form. In the event next of kin cannot be contacted for the health and well-being of my child (or myself), I hereby authorize the Administrator, Director, or Teacher of Dancer’s Pointe to authorize whatever medical treatment may be necessary in an emergency situation. I understand that I and my medical insurance carrier are financially responsible for any medical treatment extended to my child (or myself), and that Dancer’s Pointe and its agents or representatives cannot be held accountable or liable for such medical treatment.
