The above named registrant acknowledges that the Tap, Ballet, Jazz, Acro, Hip-Hop, Dance team Prep, Aerial Silks, Stretch and any other classes and/or techniques that will be taught and applied at Joni’s Studio, Inc. will and can involve strenuous physical activity and may result in injury or damage to the physical structure of the body whether the techniques taught are properly or improperly applied and practiced by the registrant.

In consideration of the benefits derived from Joni’s Studio Inc., I, (We) do hereby agree to indemnify and hold harmless, release and discharge Joni’s Studio, Inc. and the sponsoring organization of said Joni’s Studio, Inc. of Comal County, Texas, and its agents, servants or employees, from any and all claims for personal injuries or property damage occurring to or sustained by my (our) girl or boy while participating in any one or more of the above described activities or in the act of being transported to and from said activity or activities related to the above described activities including any and all consequential damage claims which I (We) may be entitled to recover as a result of said injury or property damage claim.

By registering for classes at Joni’s Studios Inc., I hereby acknowledge that any students participating with Joni’s Dance Studio may have the opportunity to be photographed or video taped in class or at various dance events and photography shoots affiliated with Joni's Dance Studio. I consent to any and all photographs/videos taken of my child or children to be included in Joni’s Dance Studio’s publications or advertisements, including on our websites and other social media platforms. No financial compensation shall be given for use of any photographs or video. I acknowledge that Joni’s Dance Studio may choose not to use my photo or video at this time, but may do so at its own discretion at a later date. Joni’s Dance Studio reserves the right to discontinue use of photos or video without notice.

I agree that tuition is due on a monthly basis and that all tuition and all payments to Joni's Dance Studio are non-refundable. In order to withdraw my dancer, I acknowledge that I must submit a statement via email to withdraw before the 1st of the month in order to avoid further charges.
