Waiver & Release Form

I acknowledge the inherent risks associated with participating in any performing arts, dance, and gymnastics program, and I understand that my child's involvement in this program signifies my express agreement to release Shine Bright Dance Studio (SBDS) and its staff from any and all claims, actions, damages, costs, liabilities, and expenses, including legal fees and court costs, arising from my child's participation at SBDS.

I hereby confirm my understanding and commitment to adhere to all Shine Bright Dance Studio policies.

Furthermore, I understand that in order to register for classes, I must enroll in Auto Pay. Tuition for classes, camps, costumes/accessories, competitions, or intensives will be automatically processed through Auto Pay at the beginning of each month. Failure to make payment will result in removal from the roster. No refunds will be provided once Auto Pay has been initiated.

In addition, I hereby authorize Shine Bright Dance Studio to utilize photographs of my child for promotional purposes.

I have thoroughly reviewed and comprehend the contents of this waiver and release, as well as the studio policies, and confirm acceptance of these processes and policies by signing this waiver.
