My signature below releases Encore Dance Company, directors, staff, employees, and independent contractors, volunteer helpers, and landlords from any and all liability that may result from myself, my children, or any member of my family participating in dance lessons, exercise classes, rehearsals, parties, private lessons, performances, field trips, or any function sponsored by Encore Dance Company.

I agree to hold Encore Dance Company, directors, staff, employees and independent contractors, volunteer helpers, and landlords 100% harmless for any and all injury that may result from my dancer, myself, or any member of my family participating in the activities listed above. Our participation is completely voluntary.

I/we realize that participation in dance classes and activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the child and parent/guardian) assume all risks related to the use of any and all spaces used by Encore Dance Company. The child participating in dance classes has up to date health insurance and, in case of an emergency, Encore Dance Company has my permission to call an emergency response unit.

I also authorize Encore Dance Company to use photos and videos of my dancer for whom I am a legal guardian for promotional purposes. If I am signing this in my capacity as the legal guardian of a minor child, I authorize Encore Dance Company to use photos and videos of the dancer for promotional purposes.
