You must be over 18 and the student's parent or legal guardian to register for an account.

I grant permission for my child/children to participate in any and all activities connected with F.A.M.E., Fine Arts & Music Education, Inc. sponsored classes. Furthermore, I agree, in case of injury or accident to my child, to hold harmless, and to waive all claims against F.A.M.E., it's officials, staff, or anyone whom it may hire or appoint to promote or supervise said activities. It is understood that my child is insured against injury while taking part in said program. This insurance is a secondary insurance policy. In case of accident arising out of said activity, medical assistance may be administered to my child herein.
