I grant my child permission to participate in dance class at the MFM dance studio. I understand what this program involves and that the aforenamed person is in proper physical condition for participation. I will see that the participant has all required equipment and will attend class regularly. I do further hereby release, absolve, indemnity and hold harmless the MFM Dance Studio, its staff, volunteers, and any other officials against all liability, claims, losses, and expenses, including attorney fees, arising from the registrants participation or be reason of any injury or damage to any person or property occuring during said participation or from any cause whatsoever. In the event of an emergency requiring medical attention (beyond first aid) i hereby grant permission to a physician or hospital personnel designated by the MFM dance studio to attend to the aforenamed person until i may be contacted. i have read and fully understand this release waiver.

Tuition is separated into 9 equal monthly payments. The first payment is due on your first class. Each month after tuition will be due the first of the month.
A $15 late fee will be added if payment is not received by the 15th of the month.
A $25 fee will be added for all bounced checks.

Costume payment is due by Dec 5st.



Costumes will not be ordered if your account not up to date. If your child's costumes need to be ordered late due to late payment, you will be responsible for the shipping and handling fee.
