Photo/Video Release:
I give full permission to Ledwig Dance Academy to publish any photos/videos of my child(ren) to the Ledwig Dance Academy website. I grant Ledwig Dance Academy the right to use the photos in any manner including, but not limited to, the website. I understand that Ledwig Dance Academy will not be punished for any use of pictures. I also understand that if there is a particular picture on the website, newspaper, etc., I do not approve of, I can ask the studio director to remove it ASAP in written form.

Liability/Health Release:
I understand that Ledwig Dance Academy is not responsible for any accidents or injuries that occur on Ledwig Dance Academy premises. I also understand that Ledwig Dance Academy is not responsible for any accidents or injuries that occur at an event where Ledwig Dance Academy is associated with – recital, competitions, etc. I declare that the listed student above is in good health and is able to participate in dance classes. In the event that an injury does occur in dance class, I give permission to Ledwig Dance Academy to call medical services or the emergency person’s number listed on the registration form.

Behavior:
I understand that I am not to be disrespectful to any teachers, staff, or other students/parents at Ledwig Dance Academy. I understand that the owner/director of the studio has the right to dismiss a student/parent from the studio if the customer is disrespectful to any staff or other parents/students.

Payments:
I authorize Ledwig Dance Academy to enroll my child(ren), who is listed above, in dance class and I accept full responsibility for the payment of tuition for those classes. I understand that if I do not pay any of my payments on time, I will be charged a late fee and will have to pay that as well. I accept full responsibility to pay registration fees, recital fees, tuition and any other fees that are due throughout the dance year. I understand that I will not get a refund for any classes I do not show up to and any fees I pay to the Ledwig Dance Academy. I will accept full responsibility of all recital payments (costumes, tights, shoes, recital fee, etc.) if I choose to put my child in recital. I understand that if I would like to withdrawal my child from dance classes I must fill out a withdrawal form.
