I understand that dance is a physical activity that may cause injury. I hereby give my permission for my child(ren) or self afore mentioned to participate in dance classes at Backstage Studio of Dance, assuming all risks in connection therewith and further waive, release, absolves, indemnify and agree to hold harmless and forever discharge Backstage Studio of Dance, it's agents, employees, volunteers, and participants, from all actions, suits, damages, claims or judgments that may result from bodily injury, property damages, losses arising out of my child's or my participation in classes, rehearsals, workshops, and performances. I assume full responsibility for all liability in connection with such claims and for having insurance for my child or self.
Furthermore, I grant permission to Backstage Studio of Dance and it's agents and employees the irrevocable and unrestricted right to reproduce the photographs and/or video images taken of my child(ren) or me, for the purpose of publication, promotion, illustration, advertising or trade, in any manner or in any medium. I hereby release Backstage Studio of Dance for all claims and liability relating to said images and videos. I waive my right to any compensation.
