I clearly understand the risks involved with dance activity and physical exercise. By registering online, I agree to release The D.A.M. Studios, and all of their affiliates, of any liability associated with my participation.

I hereby consent and authorize The Dance Art Media Studios permission to use, copyright and publish any images in any format taken of me while in class or at special events, including community performances, shows and fundraisers. I understand that these images may be used for a variety of purposes and may appear on Dance Art Media’s promotional materials or website. I also understand that Dance Art Media will use these images exclusively.

Since anyone can download an image from the internet and make copies from printed material, I agree that Dance Art Media is not responsible for unauthorized use of images. I am aware that I am not entitled to any compensation and that the images may appear with or without my name.

This agreement releases The D.A.M. Studio - Dance Art Media Ltd. from all liability relating to injuries that may occur as a willing participant / renter of dance studio space for dance related activities.

By signing this agreement, I agree to hold Dance Art Media entirely free from any liability for injuries incurred, regardless of whether injuries are caused by negligence. I acknowledge the risks involved in dancing. I am participating voluntarily and risks have been made clear to me. Additionally, I do not have any conditions that will increase my likelihood of experiencing injuries while engaging in this activity.

By signing below, I forfeit all rights to bring a suit against The D.A.M. Studio for any reason, at any time. I will make every effort to obey safety precautions as listed in writing and as explained to me verbally. I will ask for clarification when needed.

RELEASE OF LIABILITY

I have read and understand the “Waiver of Liability” and agree to comply with all information and policies. I understand and agree that neither The D.A.M. Studio - Dance Art Media Ltd. its agents, teachers and employees shall be held responsible or made subject to any claim from injury or accident which may result in connection with any attendance at the school or any of its related functions.
