I hereby consent for my child/participant and anyone attached to this account, to attend classes/programs, rehearsals and recitals at and provided by Dynamic Dance Arts (DDA). I have read and understand the policies set in place by DDA and assume and accept on my behalf the risk of injury/aggravation of existing injuries and the hazards that pertain to the strenuous physical activity of the sport of dance.
Dynamic Dance Arts shall not be held liable for any accident and/or injury of whatever nature or sort whether resulting directly or indirectly from any activities on our premises and shall not be liable for any loss or damage whether by theft or otherwise to any articles belonging to any persons brought on to our premises. I agree to release and forever discharge (agreeing to make no claim, and not to sue) DDA, and their respective owners, members, directors, and employees with respect to any and all claims related to contracting the Coronavirus (Covid-19) or any other illness and/or injury and any loss, bodily injury, or damages associated from it which I, any family member, or my child(ren) may suffer arising from of or in any way relating to him/her being enrolled in DDA, and on or off the DDA premises. This agreement of release includes any or all claims caused or claimed to be caused in whole or in part by negligence of DDA.
I understand that I have given up substantial rights by accepting and have freely and voluntarily without any inducement, assurance or guarantee being made to me and intend my electronic acceptance to be complete and unconditional release of all liability.
