I acknowledge that there is a risk of injury involved in the activity that my child (or children) is enrolled in and I accept the risk. I agree to release, waive, indemnify and save harmless Epiphany of Dance, its officers, teachers, staff members and volunteers from claims, demands, acting lawsuits or proceedings of any kind that may arise out of participation or registration in any program, event, performance or competition. I authorize Epiphany of Dance to use photos/images of my child (or children) for Epiphany of Dance School Brochure, instruction or other advertising purposes at any time.
