I recognize that Coronavirus/COVID-19 is a contagious disease and that the CDC/health authorities suggest social distancing, mask-wearing and frequent hand washing/sanitizing as preventive measures. I recognize that Step by Step Dance Expressions LLC is taking extra precautions and safety measures to prevent the spread of this virus but there is still an inherent risk of exposure to COVID-19 that exists in public settings with people present.

Coronavirus/COVID-19 is an extremely contagious disease that can lead to severe illness and death. By entering the studio/registering for classes, we (parents/guardians and dancers) voluntarily assume all risks related to exposure of Coronavirus/COVID-19 and fully understand we are furthering exposure to this virus. I/we recognize that we may become infected with Coronavirus/COVID-19 and that this could happen due to actions, negligence or omission from myself, studio owner/director, staff/volunteers, other dancers/their families and agree not to hold accountable or liable Step by Step Dance Expressions LLC, the director/owner individually, instructors, independent contractors, attorneys, insurers, officers, and staff, volunteers and facility/premises landlord/property & building owner, other agents, etc. (herein collectively referred to as designated agents) should I/we/our families/friends/contacts, contract Coronavirus/COVID-19 or a sickness of any kind during a class, event, etc., on or off premises, of Step by Step Dance Expressions LLC.

I, the undersigned, hereby for myself, my heirs, administrators, and executors, personal representatives, etc. hereby waive, release and forever discharge Step by Step Dance Expressions LLC and all designated agents from any and all liability, negligence, claims, demands, or damages of any kind, for injury, sickness, loss, damage, expense, etc. to myself, my child(ren), property etc. both now and forever in the future, out of participation in the acrobatic/dance/exercise/yoga program at Step by Step Dance Expressions LLC. This includes any action taken, or failure to act, of the studio or designated agents, or a situation that could arise in connection to services rendered by Step by Step Dance Expressions LLC both on and off studio premises; Furthermore, I recognize that Step by Step Dance Expressions LLC and all designated agents are not responsible nor liable should I/we/our families/friends/contacts contract Coronavirus/COVID-19 or any sickness/injury.

Each time I/we enter the studio, I/we understand that if I/we or anyone in our household or anyone that I/we have had close contact with has symptoms of or has tested positive for COVID-19, we will not attend classes/events at Step by Step Dance Expressions LLC until cleared from quarantine/a medial professional. Symptoms can include but are not limited to shortness of breath or difficulty breathing, fever, muscle pain, headache, sore throat, cough, or new loss of taste or smell. I further agree that I have not travelled by plane, internationally or to an area with heavy infection within the last 10 days of date of class. If any health conditions arise, we agree to notify the studio immediately.

I understand that should the circumstances regarding Covid-19 become dangerous again or government mandate make it mandatory, we will shift back to online classes and that no refunds will be issued.

I understand the Coronavirus/Covid-19 Procedures & Guidelines and agree to follow them fully.

Informed Consent, Waiver of Liability and Authorization
I, the undersigned, for myself, my heirs, administrators, and executors, etc. hereby waive, release and forever discharge Step by Step Dance Expressions LLC, the director/owner individually, instructors, independent contractors, attorneys, insurers, officers, and staff, volunteers and facility/premises landlord/property & building owner, other agents, etc. (herein collectively referred to as designated agents) from any and all liability, negligence, claims, demands, or damages of any kind, for injury, sickness, loss, damage, expense, etc. to myself, my child(ren), property etc. both now and forever in the future, out of participation in the acrobatic/dance/exercise/yoga program at Step by Step Dance Expressions LLC. This includes any action taken, or failure to act, of the studio or designated agents, or a situation that could arise in connection to services rendered by Step by Step Dance Expressions LLC. This includes any studio-related event, on or off site, including, but not limited to, community performances, competitions, rehearsals, recitals, etc. whether caused by negligence, carelessness, weather conditions, designated agents, myself or other participants, event organizers, etc. This discharge includes but is not limited to, any actions, suits, damages, claims, or judgments, or other manner of actions, causes of action in law or equity or pursuant to statue whether known or unknown, arising from or in any way connection with participation in the programs at Step by Step Dance Expressions LLC. In the event an activity or facility appears unsafe, I will notify the person-in-charge and refuse to participate until it is deemed safe.

I agree that participation is voluntary in Step by Step Dance Expressions LLC acrobatic/dance/exercise/yoga program and certify that the student(s) is/are in proper physical condition to participate in said program according to a licensed physician and is/are physically fit and have/has no medical condition or injury which would limit or prevent full participation in programs. Furthermore, I am aware of physical limitations and agree not to exceed them and agree that participants are under no pressure to physically exert beyond capacity and that it is each participant’s responsibility to use personal judgment concerning the degree of participation and discontinue and/or rest when needed.

I understand that bodily injury, partial or total disability or paralysis or death is inherent in any physical activity and I, on behalf of myself and my child(red), knowingly and voluntarily accept that risk. This could include but is not limited to muscle strains, pulls or tears, broken or fractured bones, shin splints, heat exhaustion, heat stroke, heart attack, asthma attack, leg/knee/back/foot/arm/head/neck/etc. injury, and any other illness, soreness or injury, regardless of how it was caused, occurring before/during/after participation of myself or my child(ren) in the Step by Step Dance Expressions LLC program, on or off studio premises. I understand and agree to assume all risks which may occur including those not currently known or foreseeable. I agree that I am responsible for the actions of myself and my child(ren) while at Step by Step Dance Expressions LLC and that I nor my child(ren) will take or create risks or cause harm to self/others/property.

I understand there is not medical personnel available on location and hereby authorize Step by Step Dance Expressions LLC or designated agents to obtain medical treatment for myself/my child(ren) in emergency situations and understand that I am responsible for all medical expenses and that absence of health insurance does not make Step by Step Dance Expressions LLC responsible for payment of medical expenses and that Step by Step Dance Expressions LLC does not provide medical insurance to participants.

I agree to make all tuition, costume, participation fees on time and follow policies stated in the Studio Handbook and that I received a copy of the handbook. I take full responsibility for my/our behavior and/or any damage that I/we may cause.

I understand and acknowledge that dance is physical in nature and that to ensure proper instruction and safety, physical contact is required and necessary and that there may be physical contact between director/owner, instructors, independent contractor, staff, volunteers, all designated agents, dance participants, students, etc.

I hereby affirm that I have read and fully understand and agree with the above included in the informed consent, waiver of liability and authorization statement and am signing it at my own will on my behalf of that of my child(ren). I further state that I am 18 years of age or older, competent to sign this document and am legal parent/guardian. I agree to release Step by Step Dance Expressions LLC and its designated agents of all liabilities associated with participation.

Tuition rates for the upcoming dance year are as follows: 

Returning Student Early Registration 8/13 + 8/14 only (returning students only) $20.
Registration (until 9/23) is $35 for one dancer + $25 per sibling.
Late Registration (9/24 + after is $40 for one dancer (+$30 per sibling).

The following are the tuition rates for the upcoming dance year:
1-1 hour (55 min) class = $52
1-1.25 hour (70 min) class =$65
2-1 hour (55 min) classes = $94
3-1 hour (55 min) classes (or sibling) = $142
Each additional 1 hour (55 min) class = $41

I understand that all registration and tuition fees are non-refundable and non-transferable and that:
*If I/we have an outstanding balance from the 2019-2020 or a previous dance season, I/we will need to contact Miss Katie directly.
*Each recital costume, per dancer per class is $80 for Mini + Petite levels, and $85 for all else.
--Payment is split in two due dates:
-October 21 ($40 for Mini + Petite levels, and $45 for all else.)
-November 18 ($40 for Mini + Petite levels, and $40 for all else.)

We take photographs and videos throughout the year during classes and studio-related activities so that we can better connect with you, our Step by Step family, and the wider community.

I understand that these photos and videos may be published or used for social media and website content online, advertising, promotional, etc. purposes of Step by Step Dance Expressions LLC. I hereby give permission for photos/videos of my child(ren) to be published and understand I will not be able to protest such use or receive compensation of any type for use of these photos and/or videos regardless of number of times, purpose, manner or medium, now or hereafter.

I also agree not to post pictures and/or videos related to Step by Step Dance Expressions LLC of children other than my own, and agree to make it known to my friends, relatives, etc. not to post photos and/or videos that include children other than my own. In addition, in the event a picture of my child(ren) is posted, but not on behalf of Step by Step Dance Expressions LLC, and rather by another dancer’s parent, guardian, family, friends, individual, company, event/activity or its organizers, etc., I agree not to hold Step by Step Dance Expressions LLC, the director/owner individually, instructors, independent contractors, attorneys, insurers, officers, and staff, volunteers and facility/premises landlord/property & building owner, other agents, etc. responsible.

I authorize my email address(es), cellular number(s) or other contact information to be added to the Step by Step Dance Expressions information & marketing list and to receive information via this list and that if I would like more information I can look at the studio's website privacy policy.

I hereby affirm that I have read and fully understand and agree with the above included in the photography, website, social media, marketing consent and am signing it at my own will on my behalf of that of my child(ren). I further state that I am 18 years of age or older, competent to sign this document and am legal parent/guardian. I agree to release Step by Step Dance Expressions LLC and its designated agents of all liabilities associated with photography, videography and it’s use on website, social media, marketing and promotions, etc. and am signing this at will.
