WAIVER, RELEASE, AND ASSUMPTION OF RISK
While the Kingston School of Dance and its employees take reasonable precautions to minimize risks involved with activity in dance, this is no guarantee against injury.
I, ____ (the “Releaser”) on behalf of myself or a minor in my care, hereby acknowledge and agree that in consideration of being permitted to participate in activities organized or operated by the Kingston School of Dance:

I hereby release the Kingston School of Dance, its directors, officers, employees, Board members, agents, independent contractors, successors and assigns (hereinafter collectively referred to as the “Releasee”) from all liability, recourse, proceedings, claims and causes of action, in respect of all personal injuries which I or a minor in my care may suffer arising out of or in connection with the preparation for, and/or participation in the activity.

I, on behalf of myself or a minor in my care, hereby acknowledge and agree that:

participating in the activity defined above involves a risk of physical injury which is inherent in the nature of the activity;

although it is highly unlikely, as a result of the aforesaid risks, the Releaser may suffer personal injury;

I voluntarily assume all risks involved in the activity;

By signing this WAIVER, RELEASE, AND ASSUMPTION OF RISK I am prevented from claiming against the Releasee for any loss or damage connected with personal injury that may be sustained by me or my child/ward while participating in, or preparing for the activity.
