REGISTRATION: Eagle Rock Dance has converted to an annual membership fee of $60 instead of the past $25 summer registration fee and $35 fall registration fee. Your annual membership fee is valid from June 1, 2024 through May 31, 2025. Your annual membership fee includes 2 holiday celebrations in your child's class at Halloween and Christmas, as well as our end of season parties in May. It also includes free admission to our annual daddy/daughter, mother/son themed dance and full access to our original Eagle Rock Dance core value program. This $60 annual fee will be charged on June 5, 2024 or September 15, 2024 (depending on your date of registration) and will be valid through May 31, 2025. TUITION: You will receive 3-5 lessons a month. Payment is due to Eagle Rock Dance by the 1st of the month. We offer multiple payment options.

Payments can be made by utilizing the automatic withdrawal (AutoPay); which will avoid accruing any late fees. To use AutoPay, you must sign an AutoPay authorization form or sign up through your Parent Portal account. Once completed, tuition will be debited from your bank account on the 1st of each month; there is a 3.78% convenience fee for debit/credit cards and 0.78% for ACH checking accounts.

Payments can be made by phone using debit/credit card or ACH checking. All card payments will accrue a 3.78% convenience fee and ACH will receive a 0.78% convenience fee.

A $15 late fee will be assessed if fees are not paid within 5 days of the due date. If left unpaid, another fee of $15.00 is charged every 30 days until paid in full. Declined AutoPay and returned checks will receive a $25 returned payment fee. Dancers will be asked to sit-out during class if tuition and other fees are unpaid.

There are NO refunds on classes or tuition.

CLASS CANCELLATION AND WITHDRAWAL
Class Cancellation - If Eagle Rock Dance cancels classes due to unforeseeable events, we will provide a make up class without charge within thirty days, however; no refunds or make-up classes will be given for lessons missed due to weather, power outages, or personal circumstances. IF SCHOOL IS CANCELLED FOR WEATHER, THERE ARE NO DANCE CLASSES THAT DAY.
Class Withdrawal - Inform our office staff in person if withdrawing. AutoPay will stop after a one-month notice period. NO credit will be given if dancers withdraw from class after the first of the month. We reserve the right to terminate lessons to any students without notice. In such a case, a refund for unused lessons will be given.
Quit Fee-If on an applicable team a quit fee may apply if dancer withdraws midseason as per signed agreement. See your copy of signed agreement.

HANDS ON CORRECTION POLICY
We use hands on corrections. If you would prefer your child not be touched, or your child would prefer not to be touched, THAT MUST BE COMMUNICATED IN WRITING TO THE STUDIO AND ADDED TO YOUR CHILD'S FILE.

PHOTO RELEASE
Eagle Rock Dance has permission to take photographs of the student to use in brochures, web sites, posters, advertisements and other promotional materials. Permission is also granted for the studio to copyright such photographs in its name.

INSTRUCTOR SUBSTITUTIONS
Coach Substitutions: Eagle Rock Dance reserves the right to provide a substitute instructor if the regularly scheduled instructor is ill or otherwise unable to attend classes. If an instructor is ill and we cannot arrange a substitute or a make-up class, any missed classes will not be charged.

PARENT RESPONSIBILITIES
Parents and Children: Eagle Rock Dance is not responsible for children not registered in class. Parents with dancers under the age of 5 should remain at Eagle Rock Dance during that child's lesson.
Eagle Rock Dance does not provide child care. Parent(s) are responsible to ensure their child(ren) is/are supervised, make it into each class they are scheduled for, they do not disturb students, classes, or instructors, the facility and its equipment are respected, and equipment is only for instruction. Parents are to clean up after their child(ren).

RISK OF INJURIES
I fully understand that Eagle Rock Dance staff members are not physicians or medical practitioners of any kind. With the above in mind, I hereby release the Eagle Rock Dance staff to render temporary first aid to my child or children in the event of any injury or illness, and if deemed necessary by the Eagle Rock Dance staff to call a doctor and to seek medical help, including transportation for said child should the Eagle Rock Dance staff deem this necessary.
Injuries: Parents, legal guardians of minor students and adult students waive the right to any legal action for any injury sustained on Eagle Rock Dance property resulting from normal class activity or any other activity conducted by the students before, during or after class time.

WAIVER AND RELEASE
I authorize Eagle Rock Dance to instruct me/my child in a program of dance education. In consideration of this agreement to instruct and assist me/my child, I do here and forever release and discharge and hereby hold harmless Eagle Rock Dance and their respective instructors, employees, and contractors from any and all claims, demands, damages, rights of action or causes of action, present or future, arising out of or connected with me/my child’s participation in this dance program, including any injuries resulting therefrom. Eagle Rock Dance is not liable for any communicable diseases or illnesses. There will also be no refunds given for any sort of natural disaster if an opportunity is provided in some format for the dancer to continue to be able to participate in class. I understand it is my responsibility to maintain medical coverage for myself/my child, and I authorize Eagle Rock Dance to obtain emergency medical treatment for me/my child for injury or illness while attending dance classes, performances, competitions, or activities. I also authorize Eagle Rock Dance to use my child’s picture for promotional reasons.
