You must be over 18 to register for an account. If you are under 18 years of age, your parent must create the account.

I hereby release MVMT Foundation from any and all liability, claims, demands, causes of action and possible causes of action whatsoever arising out of or related to any loss, damage or injury that may be sustained by myself or child while participating in or traveling to and from this event. I also authorize MVMT Foundation to seek and consent to emergency medical attention for myself or child as needed; and I further agree to be liable for and to pay all costs incurred in connection with such medical attention.

I hereby agree and consent to all terms listed in the Dancer Contract document provided by MVMT Foundation and located within the Share Files in Dance Studio Pro.

I authorize MVMT Foundation to use the following personal information:

(1) My or my child's image and likeness- including photographic, motion picture, and electronic video images.
(2) My or my child's voice- including sound and video recordings.

I hereby waive the right to receive any compensation for MVMT Foundation use of any of the material described above for any of the purposes authorized by this release.

I agree to accept full responsibility, financially or otherwise, for any damage I may do to the property of MVMT Foundation (MVMT Dance Studio), which includes its employees, members, and all other persons involved.

By enrolling in our autopay system for tuition payment, you acknowledge and agree to the following terms:

1. Consent to Information Collection:
Enrolling in autopay authorizes The Diamond MVMT to collect, process, and securely store your payment information on file for all outstanding fees.

2. Opt-Out Option:
Parents have the right to opt out of autopay by submitting a helpdesk ticket using this link: https://airtable.com/appXm826EiPtg96Yc/shrohI9qNizpwUJBy indicating their preference not to use autopay for tuition payments.

3. *Payment Capture*: Autopay will capture only the most recent tuition and outstanding balance posted at the time the autopay runs. In the event of a failed autopay transaction, members are responsible for manually settling any outstanding balances.

4. Autopay Incentive:
Members who opt to use autopay for tuition payments will receive a credit [TBH] applied to their account.

By opting into autopay, you understand and agree to these terms, acknowledging that the responsibility for maintaining updated payment information and addressing any failed transactions lies with the member.

By participating in our program, members agree to the following withdrawal notification terms:

1. Notification Requirement:
Members are required to submit a helpdesk ticket as the formal notification method regarding their decision to withdraw from the program. The helpdesk ticket should include relevant details and be submitted through the designated platform or channel provided by The Diamond MVMT.

2. Open Discussion and Support:
Upon receiving the helpdesk ticket notification, our leadership is committed to initiating open discussions and providing necessary assistance during the withdrawal process. We are available to address concerns, offer support, and provide guidance as needed.

3. Settlement of Balances:
Upon withdrawal, members are responsible for settling all remaining balances that cover past attendance. These balances must be cleared within the agreed timeframe after the withdrawal notification.

This agreement ensures that both parties maintain clear communication and responsibility, allowing our leadership to promptly assist members upon receiving their withdrawal notification while ensuring the settlement of all outstanding balances covering past attendance.

Refund Policy:
We uphold a strict no-refund policy for tuition payments.

Continuous Access:
Once payment for a month of tuition is made, the member retains the right to attend classes throughout that month.

Withdrawal and Proration:
Should a member choose to withdraw, fees will not be prorated, but they are encouraged to complete the ongoing month's classes.

-Billing on Last Date of Attendance:
In cases where withdrawal notification is delayed, billing will be based on the last attended classes for the entire month.

-Exception Handling:
Exceptions to this policy will be evaluated on a case-by-case basis and require approval from specific authorities: Executive Director (ED), Director of Operations (DO), or the Founder.

Empathy and Flexibility:
Unforeseen circumstances will be considered with empathy, and flexibility will be exercised within the confines of the policy.

This agreement assures fairness and consistency while allowing for exceptional circumstances to be considered on an individual basis.

I acknowledge the Spring 2024 schedule for the following groups:

I understand that this schedule serves as the foundation for the season. Additional rehearsals may be included to ensure optimal preparedness for live performances and visuals. Any additional rehearsals will be communicated in advance to facilitate necessary adjustments.

I acknowledge that the following circumstances constitute excused absences:

a.Death in the family
b.Scheduled school programs, classes, or performances necessary for credit
c.Personal or family weddings or religious dedications
d.Instances of mental, or physical illness, or contagiousness
e.Pre-planned events communicated with a minimum of two (2) weeks’ notice
f.Fulfillment of financial obligations


I understand that misusing these excused absences may lead to disciplinary measures.

Monthly tuition covers comprehensive dance training. Participation in live performances and visuals is based on auditions.

Keep in mind, that auditions are not the sole determining factor. Our directors consistently assess dancers' progress, learning style, and overall performance during training, which influences these decisions.

It is important to note that commitment to The Diamond MVMT training program does not automatically ensure participation in live performances and visuals.

The Diamond MVMT Leadership retains the authority to choose members for live performances and visuals. Further details about performance opportunities will be provided by performance creative directors, choreographers, and captains throughout the season.

I acknowledge that I have reviewed the Fall 2024 Handbook in its entirety that can be found under the "Files" section within my Dance Studio Pro Portal and pinned to the top of the "Inside the MVMT" BAND.

Upon review of the Fall 2024 Handbook and waivers listed within Dance Studio Pro:

1). I understand and agree with the expectations regarding class schedules, attendance, and participation;

2). I understand and agree with the expectations regarding communication with leadership and the use of BAND; and

3). I understand and agree with the expectations regarding the financial commitment to participate. Furthermore, I understand that there are due dates, an assigned payment method, and that communication with leadership is expected in the event that payment falls behind.

Lastly, I understand that failure to provide payment or communication will lead to termination of participation in performances and/or rehearsals.
