I hereby release The Diamond Dancers Company, LLC and MVMT Dance Studio from any and all liability, claims, demands, causes of action and possible causes of action whatsoever arising out of or related to any loss, damage or injury that may be sustained by myself or child while participating in or traveling to and from this event. I also authorize The Diamond Dancers Company, LLC to seek and consent to emergency medical attention for myself or child as needed; and I further agree to be liable for and to pay all costs incurred in connection with such medical attention.

I hereby agree and consent to all terms listed in the Dancer Contract document provided by The Diamond Dancers Company, LLC and located within the Share Files in Dance Studio Pro.

I authorize The Diamond Dancers Company, LLC to use the following personal information:

(1) My or my child's image and likeness- including photographic, motion picture, and electronic video images.
(2) My or my child's voice- including sound and video recordings.

I hereby waive the right to receive any compensation for The Diamond Dancers Company, LLC use of any of the material described above for any of the purposes authorized by this release.

I agree to accept full responsibility, financially or otherwise, for any damage I may do to the property of The Diamond Dancers Company, LLC (MVMT Dance Studio), which includes its employees, members, and all other persons involved.

The following are considered excused absences:
-death in the family,
-scheduled school programs, classes, or performances required for credit,
-wedding or religious dedication of you or your family,
-if you are (mentally or physically) ill, infectious, or contagious,
-event planned with notification provided two (2) weeks or more in advance, and
-financial obligations.

Abuse of these excused absences can and will result in disciplinary action.

I acknowledge that I have reviewed the Fall 2023 Handbook in its entirety that can be found under the "Files" section within my Dance Studio Pro Portal and pinned to the top of the "Inside the MVMT" BAND.

Upon review of the Fall 2023 Handbook and waivers listed within Dance Studio Pro:
1). I understand and agree with the expectations regarding class schedules, attendance, and participation;
2). I understand and agree with the expectations regarding communication with leadership and the use of BAND; and
3). I understand and agree with the expectations regarding the financial commitment to participate. Furthermore, I understand that there are due dates, an assigned payment method, and that communication with leadership is expected in the event that payment falls behind. Lastly, I understand that failure to provide payment or communication will lead to termination of participation in performances and/or rehearsals.

Monthly tuition covers comprehensive dance training. Participation in live performances and visuals is based on auditions. Keep in mind, auditions are not the sole determining factor. Our directors consistently assess dancers' progress, learning style, and overall performance during training, which influences these decisions. It is important to note that commitment to The Diamond MVMT training program does not automatically ensure participation in live performances and visuals. The Diamond MVMT Leadership retains the authority to choose members for live performances and visuals. Further details about performance opportunities will be provided by performance creative directors, choreographers, and captains throughout the season.

Please note that the schedule listed above is the base schedule for the season. Additional rehearsals may be added to ensure adequate preparation for live performances and visuals. The need for these additional rehearsals will be determined during the season and communicated in advance to allow for the necessary adjustments.
