If my emergency contact cannot be reached, I give permission to the staff of Strut Your Stuff of Plainview, in. to render aid or to act on my behalf to obtain emergency medial treatment for the student for any illness or injury that may occur while attending classes, rehearsals, performance on or off premise. I confirm that I will not send my child to dance class if they or anyone in their household has symptoms of Covid-19 or similar infectious disease. Furthermore, I agree to the studio policies put in place by Strut Your Stuff of Plainview, inc. and understand there are no exception of the rules for myself or my dancer. Payment information: There is a non refundable fee of $30/individual or $50/family. This is due at time of registration along with your first tuition payment. SYSdance tuition is based on an ENTIRE SEASON of dance. Tuition is not based on the number of classes per month or by your attendance. There is NO refund for missed classes as make up lessons are an option. Seasonal Investment: Options are avialable for your payment schedule. Payment is due by the tenth of the month. Three Options for payment: #1: Entire season paid in full by 9/20 ( 5% discount for cash payment) #2: Four payments: 1 due at registration with 3 equal installments due by the tenth of Oct/January/April #3: For students taking 4 or more classes - pay 1st with registration, monthly installments Sept-May *In the event that we accept credit card payments, the cc fees will be added to your tuition payment. Cash, check or Zelle will not incur fees. Costume payments are non-refundable. Recital participation is optional. Deposit of $50 per class is due 10/23/23. Balances due by 12/9/23 Tuition payments - No refunds after 1/10/24 Year end performance participation requires a fee of $130 which includes two performance tickets(show of choice), one 2024 performance shirt and the digital download of the entire performance. This fee will be due in full by 2/10/2024 Dress Codes: parents agree to purchase the required dance wear and proper dance shoes for the class(es) their dancer is participating in no later than mid-October. If your dancer drops a class, a 14 day written notice is required. You will be responsible for payment until the written notice is received and the 14 day period has passed.
