You must be over 18 to register for an account.

I consent to the above mentioned student participating in lessons with and at Radiance Academy Toowoomba. I also agree to indemnify and keep indemnified Radiance Academy Toowoomba, the staff, teachers and contractors from and against all actions, costs, charges and expenses whatsoever which may be brought or made or claimed against them or any of them arising from the dance classes at Radiance Academy Toowoomba.
I understand and agree that in participating in any dance class, workshop, rehearsal or performance there is the possibility of a physical injury. I agree therefore, to assume all risks of any such injury to myself or my minor child, which may occur during any and all of Radiance Academy Toowoomba events, classes or performances.
I also agree to assume all risks of any nature to myself or minor child when travelling to and from the above mentioned. I release and indemnify Radiance Academy Toowoomba from any and all liabilities and claims whatsoever from any damage, loss or injury to me or my minor child or our personal property which may have arisen out of or in connection to participation in any dance class, workshop, rehearsal or performance conducted by Radiance Academy Toowoomba.
I also agree that by registering for dance lessons, I will pay for these dance lessons within the time period specified on the account. If I wish to cancel lessons prior to the close of the calendar year I will give 2 weeks notice in writing.
