By checking this box, I acknowledge that this activity has inherent risks for the participants listed above and I release Dance Hues Studio and any volunteers working on Dance Hues behalf from all liability, costs, and damages which could arise from attending any Dance Party and Parent's Night Out events for which I registered my student. I agree to accept financial responsibility for costs related to any emergency treatment incurred.
