ACKNOWLEDGEMENT OF RISK, WAIVER OF LIABILITY, AND INDEMNIFICATION - I hereby consent to participating in (or my child's participation in) A New Pointe Dance LLC/ Dance Programs. I recognize that potentially severe injuries can occur in any activity involving height or motion, including leaping, spinning, jumping, stretching, and other related dance movements. In consideration of allowing my child to use these facilities, I hereby forever release A New Pointe Dance LLC, its instructors, its employees, or any volunteers from any and all liability, for any and all damages, and for injuries suffered by my child while under the instruction, supervision, or control of A New Pointe Dance, its instructors, its employees, and other volunteers.
