Payment Responsibility

I (You) am responsible for payment of fees for all services rendered or ordered (registered for) and all merchandise delivered to you or ordered by you in accordance with DU payment policies. In consideration of the benefits derived from DU, I (we) do hereby agree to hold harmless Dance Unlimited MA, Ware, Massachusetts, it's agents or employees, from any and all claims for personal injuries to myself and/or any of my dependent children while participating in dance classes or any other DU activities, or while in the act of being transported to and from said activities related to dance or other DU activities. DU is not responsible for your personal property including automobiles and personal items located in our parking lot or facility. DU is not responsible for the supervision of your dependent children located in our designated waiting area, facility grounds, parking lots or any other location(s) in which they are not receiving registered dance instruction.
